Partnership for Strong Families is the lead agency for a community-based network in 13 counties in North Central Florida, providing services to ensure the safety, well-being and security of its children and families through foster care and related services. We are seeking an Integrated Practice Team Facilitator to join our team in Gainesville, Florida. This is a highly responsible position in charge of coordinating and facilitating Integrated Practice Team (IPT) Conferences. This position involves the facilitation of IPTs, as well as the evaluation and decision making processes discussed within the context of the IPT. This position acts as a resource person for the Child Protective Investigator and/or Family Care Counselor, as well as an array of community partners and will be directly supervised by the Clinical Project Manager.


The Integrated Practice Team (IPT) is a community-oriented, strength based individualized planning process aimed at helping people meet their unmet needs both within and outside of formal human services systems. A team of community partners who offer knowledge and expertise to develop a comprehensive family service plan with innovative ideas to ensure the individual needs of the family are met while safely maintaining children in their homes.


The Parent Needs Assistance (PNA) program processes reports received by the DCF Hotline in situations where parents/ caregivers are seeking assistance for their family and no safety concern is identified.  These reports are processed and contact is made with the families to help them link to community resources.

Responsibilities include:

  • Research relevant case information to adequately prepare for IPT conferences and identify potential attendees, including staff, service providers, family members and natural supports.
  • Coordinate IPT conferences by considering the schedules of all attendees and reserving a location for the conference that is safe and allows for open dialogue.
  • Ensure appropriate evaluation and decision making processes take place within the context of the IPT.
  • Ensure appropriate triage and evaluation for families as assigned within the PNA program. Follow-up with assigned PNA cases, linking families to suitable community resources.
  • Facilitate IPT conferences to engage the family and assist in the development of an appropriate follow up plan to prevent entry or deeper entry into the formal child welfare system.
  • Display understanding of, and sensitivity to, service population’s cultural and socioeconomic characteristics.
  • Work in collaboration with the Family Advocate, employed by the Department of Children and Families.
  • Provide crisis intervention linkages and support through consultation to ensure family support and placement stabilization.
  • Participate in meetings/staffings and coordinate activities between various stakeholders within the System of Care.
  • Foster relationships with service providers, hospitals, courts, education system, Juvenile Justice System and Domestic Violence and Substance Abuse & Mental Health agencies.  


The ideal candidate will have a Bachelors’ degree in Psychology, Social Work, or related field with 3 years of experience in mental health, group facilitator, or family counseling. A valid Florida Driver’s License, an acceptable driving record, and access to reliable transportation is required.


Preferred qualifications include: a Master’s degree in a related field, experience with the Family Group Decision Making model, experience working with at risk families and children with significant mental health and behavioral needs, and prior DCF experience or knowledge of child welfare systems, data management systems, and wraparound planning.
