This position reports to the Chief Operating Officer (COO) and closely collaborates with the Medical Director for Migrant Health Services and other FHLC leaders, to optimize access to services for Wisconsin’s Migrant and Seasonal Agricultural Workers (MSAW) and their families in order to achieve high-quality clinical outcomes.  This position also works to optimize access to services for the service area’s dairy workers and other under-served populations who are unable to easily access services.  The Director ensures service line fiscal and operational optimization. Responsibilities include planning, directing, and evaluating FHLC’s special population outreach services and maintaining and developing new strategic partnerships that support improving access to services.   The Director is accountable for the operational and fiscal performance of MSAW outreach operations and works to ensure the efficient performance of the outreach services team.

• Plans, directs and evaluates FHLC’s special population outreach services including planning and implementation of MSAW service line and related specialty and ancillary service lines.
• Develops and monitors the service line goals, programs and objectives, personnel, resources, and equipment/supplies.
• Supports the team focus in providing excellent patient care
• Represents FHLC’s mission, vision and values and utilizes and models an appreciation of and respect for justice, equity, diversity, & inclusion in every interaction.
• Researches, develops, and implements innovative and best practice models to increase access to care and health outcomes for WI MSAW population and other under-served populations in FHLC’s service area.
• Supports and advocates the advancement of care and services available to the MSAW community and other under-served populations.
• Creates and maintains collaborative relationships with other health systems and community agencies to promote FHLC mission and services and develops/maintains strategic partnerships that support improving access to services.
• Presents information about FHLC, MSAW and other special populations to various groups to promote health equity, understanding, and improved health outcomes. 
• Ensures monitoring of providers’ schedules to facilitate and expedite patient access and timely patient care.
• Ensures that quality improvement infrastructure and methods are embedded into medical and ancillary operations and conducts and ensures quality assurance testing
• Ensures that mobile health staff comply with state, federal, and other governing rules, regulations, and codes.
• Encourages and demonstrates cooperation among providers, medical assistants, support staff, and relevant departments
• Partners with all staff to emphasize effectiveness, efficiency, sustainability, patient-centeredness, and staff engagement at every juncture.
• Meets with clinical and operational teams about outcomes data to discuss implications for clinical practices and identifies areas needing improvement.
• Works with the COO, Chief Medical Officer, and direct reports to optimize staffing and appointment schedules, redesign workflows as needed, and create and monitor policies and procedures, as needed to meet goals.
• Performs all responsibilities, in accordance with organizational policies and procedures, and ensures compliance with HIPAA Privacy and Security Standards.
• Promotes a culture of high performance and continuous improvement that values learning and a commitment to quality.
• Responsible for the recruitment, hiring, and retention of staff as needed to support patient demand and staff needs.
• Ensures appropriate development and training of department staff
• Collaborates with other service line directors and staff to ensure integration of services and care in alignment with the FQHC model of care
• Works with finance team to create and operate according to departmental budget plans
• Ensures all risk management policies and procedures are followed and appropriate contingency plans are developed to mitigate risk.
• Together with other leaders, supports the ongoing administration of FHLC’s Emergency Preparedness plan and may represent FHLC at regional Emergency Readiness Coalitions and planning groups.
• Facilitates and support process improvement initiatives.
• Develops and presents reports related to performance, activity, progress, compliance, etc., for the service lines under their purview and assists with report development for other FHLC performance initiatives.
• Maintains open channels of communication with all FHLC senior leaders
• Ensures problems/issues are identified and resolved in a timely manner. 
• In collaboration with the Director of Quality and Compliance submits written recommended actions to address root causes of complaints, compliance issues, and system break-downs.
• Must adhere to FHLC policies and procedures.

Bachelor’s Degree in a relevant field or equivalent experience required.  Master’s degree or equivalent experience preferred.  Demonstrated experience and understanding of the needs of migrant and seasonal farmworkers and their families is required. Lived experience highly preferred. CPR/BLS certification required within six months of hire.

Bilingual English and Spanish language required.
