Position Summary:

Mainstay Supportive Housing & Home Care is seeking a kind, friendly, thoughtful, caring, and dependable candidate to work Per Diem as a Home Health Aide (HHA). The position involves providing one-to-one supportive services to clients in Chelsea, MA. Services are provided in the client’s apartment. Clients have varying degrees of independence and a variety of support needs, including for aging in place, mental health issues, and physical disabilities.

Duties and Responsibilities:

  • Have upbeat, genuine interactions with the clients, staff team, property management team, other service providers, and family members
  • Perform Direct Care duties guided by the client’s Plan of Care. These duties focus heavily on individualized support (including cueing and hands-on support) of Activities of Daily Living. (Activities of Daily Living include showering, shaving, mouth care, hair care, skin care, dressing, exercising, toileting, medication reminders, ambulation, and transfers.)
  • Assist client with laundry, light housekeeping, light grocery shopping
  • Light meal prep for/with client, following dietary plan if applicable
  • Provide companionship to the client, doing something mindful and fun or recreational during daily visits (e.g. playing cards, going for a walk)
  • Files and Record Keeping: With support and training from supervisors, write daily service logs, track daily goals of the Plan of Care, record service delivery on the GPS system.
  • Report any client health status changes to supervisor
  • With support and training from your supervisor, learn the procedures and policies of the MassHealth Group Adult Care Program (GAFC) — no prior experience with this program is expected.
  • Complete required online monthly training and attend required meetings (paid time). (Currently all meetings are remote, but this is subject to change.)
  • The ideal candidate will be a team player, have excellent relationship-building skills, and will collaboratively work with families, fellow staff, nurses, other service providers, and the management team to ensure that clients are receiving appropriate individualized services and maintaining connections in the community.
  • Adequate writing/technology skills to communicate in writing, complete incident reporting and to ensure files and records are kept up to date. 


Education:  High School Diploma, 1-3 years of experience working with individuals with a variety of disabilities, preferably in a residential setting.

Licensures/Certifications: OPTIONAL: CPR and First Aid certifications (or ability to pass within the first 30 days of employment).
