JOB SUMMARY

        Health Federation of Philadelphia is launching a new care management program aimed at reducing health inequities, addressing social determinants of health, and improving health outcomes for low-income patients struggling with chronic physical and mental health conditions. We are seeking an experienced Community Health Worker who is familiar with community resources in Philadelphia.

       

      CHWs serve as helpful caring peers supporting their clients to manage challenges by connecting

      clients to social services, behavioral and clinical care, and helping with a variety of social issues that

      may be in the way of clients staying healthy.

 

This position will be part of a team of four, including the Team Manager (a Social Worker), a nurse Care

Manager and another Community Health Worker who will work with six community health centers

and a Medicaid Managed Care Organization (MCO) to achieve improved outcomes for patients.  The

Team will work to assess needs of identified patients at high risk of poor health outcomes, and to

engage these individuals in primary care and other services, with the goals of improving health status

and reducing hospitalizations, as well as to address quality metrics and “care gaps”.  The Care

Management Team will engage patients who are not receiving regular care at their assigned primary

care location and follow-up with individuals with recent hospital contact (inpatient and/or Emergency

Department).

 

The two Community Health Workers will serve as experts on community resources and as a bridge between patients and the health care system.  This position is a mix of telephonic and face-to-face patient contact; in-person outreach may take place at local hospitals, community health centers, and/or patient homes.

 

JOB SPECIFICATIONS

Responsibilities/Duties:

  • Engage patients to participate in Care Management services, to understand and address social barriers to good health, and to reinforce self-management for positive health outcomes, via phone and in-person outreach
  • Connect and guide patients to community resources and assist patients in navigating the healthcare system.
  • Support development and implementation of a care management plan that is mutually agreed upon by the health care team and the patient/family.
  • Perform follow up calls for patients recently discharged from the hospital. As needed, perform hospital visits and/or home visits.
  • Assist patients to ensure attendance at primary and specialist care, mental health and other appointments. Communicate effectively with health care providers and staff at these locations.
  • Maintain accurate and timely documentation.

Education:

H.S. Diploma/GED required

CHW training and certification preferred

Be eligible to apply for the Grandparenting application to be a certified CHW with the PA Certification Board

 

 

Required Qualifications/Skills:

  • Past work experience as a Community Health Worker (or similar role)

  • Demonstrates passion to help your communities through informal experience, including helping neighbors or family members

  • Demonstrates interpersonal skills to interact effectively with healthcare providers, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.
  • Demonstrates ability to function effectively in a fluid, dynamic and rapidly changing environment.
  • Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.
  • Demonstrates familiarity and comfort with technology and data, including electronic health records, payer portals, Microsoft office, and Google apps.
  • Demonstrates ability to work autonomously in the community, while maintaining professional boundaries and productivity.
  • Knowledge of basic medical terminology

Preferred qualifications:

  • Resident of one of the communities served by the program (Philadelphia, Chester, Pottstown) and knowledgeable about resources within these communities.
  • Bilingual in Spanish

 

Position Type and Work Schedule
This is a full-time, non-exempt (hourly) position. 

Work Environment
Standard office setting, plus regularly scheduled meetings, trainings, etc. with participating health centers, occasional home or hospital visits.

 

Physical Demands
Able to sit and work at a computer keyboard for extended periods of time.  Able to lift and move up to 15 pounds.

 

Travel
Must be able to get around effectively within Philadelphia and surrounding counties, either on public transportation and/or in their own vehicle.  Limited travel beyond surrounding counties will be required, as well.

$19.50-$21.50 hourly depending on experience and qualifications
