POSITION SUMMARY:

The Outreach Navigator will  work specifically with individuals who are experiencing long-term, chronic homelessness and have an opioid disorder.  The Outreach Navigator will be responsible for helping individuals obtain housing, referrals to appropriate resources, assist with access to  basic needs, support individuals with substance use  or mental health treatment goals, and establish  medical care through progressive engagement. The Outreach Navigator will be community based, supporting people in their current environment which may include streets, under bridges, in parks, camping, etc.  The ideal candidate for this role thrives in an unpredictable work environment.  Our housing teams utilize Housing First and Harm Reduction models to assist individuals who have experienced long-term homelessness to find and maintain housing.

ESSENTIAL RESPONSIBILITIES:

  • Provide Outreach services to those experiencing literal homelessness, unstable housing, and those unable to be inside during daytime hours, connecting them to supportive services and ensuring their basic needs are met. 
  • Coordinate with Avivo Supportive Housing programs and other service providers to help clients achieve stability. 
  • Accurately complete paperwork in a timely manner.  This will include:  case notes, data collection and monthly reporting for assigned clients utilizing online databases including HMIS and Procentive.   
  • Respond to voicemails, emails, and other communication promptly.
  • Verbal and written communication is clear, effective, and contains adequate detail.
  • Demonstrates appropriate professional boundaries with clients and colleagues.
  • Diligently protect client’s data according to HIPAA standards.
  • Collaborates effectively with team members, other programs, and external organizations.
  • Seeks out professional growth, training opportunities, and additional tasks.
  • Maintains a professional attitude in the face of change, challenging/stressful situations/people.
  • Understands and adheres to mandated reporting responsibilities.
  • Use personal vehicle for outreach activities.

 

 

 

QUALIFICATIONS:
  • Associate, Bachelor’s or Masters degree in a related field.  Licensure helpful. 
  • Knowledge of, or experience in the provision of services to a predominantly Native American community, impacted significantly by Opioid use. 
  • Committed to the delivery of services that are in alignment with Housing First and Harm Reduction philosophies. 
  • Hold the belief that every individual deserves housing and can achieve housing stability.
  • Willingness to engage in outreach, outdoors, to include during times of extreme heat or cold.
  • Ability to relate to and advocate for diverse client needs and interests.
  • Ability to learn and understand client needs and mental health recovery related to the provision of community mental health services.
  • Knowledge of County Coordinated Entry system and VI-SPDAT assessment tool (preferred)
  • Strong written and verbal communication skills and computer experience.
  • Maintains access to a reliable vehicle with valid insurance  and ability to drive for work.
