Recovery Network of Programs, Inc. (RNP), is a private, non-profit, social service agency which has been serving the Greater Bridgeport Community and Fairfield County area since 1972. RNP is dedicated to helping those living with substance use, mental health disorders, and homelessness. Optimus/RNP Primary Integrated Care (PIC), a 3-year SAMHSA grant program is on-site at KTC to provide MAT clients with immediate access to Primary Care. PIC offers integrated, bi-directional care for primary and behavioral health.

Hours: Monday - Friday 9am to 5pm


  • Acts as a liaison between the PIC program, Optimus Health Care and RNP programs by responding to requests for outreach and support
  • When needed, assists with informing clients, referral sources and friends/family about program-specifics and responds to questions in a professional manner with emphasis on easing anxiety, building trust and confidence and offering clarity.
  • Completes standardized assessments such as the National Outcomes Measurement System (NOMS). May also need to conduct the following screenings: PHQ-9, GAD, PTSD-PC and SBIRT for the presence and absence of substance abuse and mental health symptoms.
  • Assists with referrals to the PIC program including scheduling evaluations, completing NOMS (baseline, re-assessment and discharge)
  • Conducts biopsychosocial evaluations and develops an initial treatment plan that identifies the needed medical, mental health and/or substance use services including MAT (medication assisted treatment)
  • Assist with completing required quarterly and yearly data reports to DHMAS along with Optimus on site supervisor
  • Immediately notifies the Optimus on site supervisor and RNP clinical supervisor if a patient is at risk of hurting themselves or others.
  • Actively participates in clinical team meetings to coordinate admissions and discharges.
  • Compiles admission and discharge information and creates a monthly report and submits to Optimus Health Care on site supervisor and Director of Grants and Innovation.
  • Participates in monthly Integrative Care Review Team meetings which includes working with the PIC team to identify a patient, complete document for care team review and presenting information to the team
  • Coordinates physical exams and necessary medical appointments with Optimus physicians.
  • Ability to utilize identified screening/assessment tools and stages wise interventions & treatment models.
  • May conduct treatment utilizing best practices of counseling, case management and crisis intervention.
  • Assumes all other appropriate duties as deemed necessary by Optimus on site supervisor and RNP clinical supervisor.


  • Masters Level Degree in social work, counseling, psychology or related field.
  • Licensed or license eligible (LCSW, LMFT, LPC, LMSW).
  • Two (2) Years related experience in providing assessment and behavioral health services required (direct client care internship experiences applicable).
  • Knowledge of motivational interviewing and stages of change.
  • Excellent organizational and communication skills, both written and verbal.
  • Maintain electronic health records in accordance with state and federal laws
  • Excellent computer skills in excel and word.
  • Ability to manage multiple issues at the same time and prioritize competing demands or switching direction quickly.
  • Highly methodical and process oriented.


  • Incumbents may be required to secure professional certification and/or license.
  • Valid Connecticut driver's license may be required.
