Recovery Network of Programs, Inc. (RNP), is a private, non-profit, social service agency which has been serving the Greater Bridgeport Community and Fairfield County area since 1972. RNP is dedicated to helping those living with substance use, mental health disorders, and homelessness. Our mission is to restore hope, health, and wellbeing for individuals and families in a recovery environment that embraces compassion, dignity and respect.

Prospect House This program provides emergency shelter to thirty-six adults daily in a facility located in Bridgeport, Connecticut. The program provides comprehensive case management services to meet the varied and complex needs of individuals who are homeless, and work to connect clients to a long-term housing solution. The case management program at Prospect House includes assessment, transportation, life skills training, as well as individual, group, and family sessions.

Hours: Monday - Friday 12:00pm - 8:00pm and Rotating Weekends

CORE FUNCTIONS

  • Conduct outreach and organizational in-reach to identify households who are experiencing homelessness.
  • Assess the individuals/familys safety, immediate needs, financial assistance, level of functioning, overall mental and physical health and strengths and capacities.
  • Assertively provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
  • Provide crisis intervention for homeless individuals and families in the community.
  • Assesses and records medical, psychological, and social factors contributing to client's situation and evaluates these capacities.
  • Links individuals to community resources and other natural supports.
  • Develops and documents person centered plans.
  • Maintain timely progress notes which record and monitor client progress housing, finances, employment, education, nutrition, basic needs, physical health, mental health and substance abuse related issues.
  • Serve as advocate for clients in obtaining necessary Housing resources and services.
  • Adhere to all standards as it relates to documentation requirements.
  • Attend and be prepared for case management and staff meetings.
  • Prepare supervision agenda.
  • If appropriate, assume on-call responsibilities.
  • Maintain work schedule which allows for monthly weekend rotations.
  • Assumes all other appropriate duties as deemed necessary by the supervisor.

CORE COMPETENCIES

  • Bachelor's degree in the behavioral sciences and two (2) years experience in counseling preferred; or equivalent combination of education and experience. Co-occurring competencies and or credentials are preferred.
  • Knowledge of the physical, psychological, social and economic effects of homelessness, substance abuse, physical and emotional illness, crisis intervention.
  • Knowledge of, or ability to quickly develop competency with, issues related to homelessness, homeless prevention, effective intervention techniques, and basic understanding of mental health and substance abuse issues
  • Knowledge of principles and techniques of social service case management services.
  • Must be able to work effectively with populations who have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy, and numeracy issues, and/or other conditions or situations that have impacted housing stability.
  • Strong organization skills and ability to coordinate and complete multiple tasks simultaneously
  • Ability to gather, analyze and evaluate information pertinent to client care.
  • Knowledge of community resources.
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality.
  • Ability to establish and maintain cooperative professional relationships.
  • Excellent communication skills.
  • Ability to project a professional image.
  • Ability to work with diverse populations.
  • Ability to work independently or with a group.
  • Assertively link individuals to housing and community resources.
  • Ability to engage local landlords, explain subsidies process and advocate for landlord participation.
  • Ability to coordinate multiple tasks simultaneously.
  • Highly motivated, organized, problem solver, creative with excellent interpersonal skills.
  • Bilingual/Bicultural preferred.

CERTIFICATES, LICENSES

  • Incumbents may be required to secure professional certification and/or license.
  • Valid Connecticut driver's license is required.
