POSITION SUMMARY:

The Housing Case Manager will be working specifically with individuals that are at Avivo Village, Avivo’s newest housing program.  Avivo Village works to house unsheltered individuals in a temporary tiny home with the hopes of securing long-term housing within 90 days. This program serves single adults and couples who have a history of long-term homelessness.  The Case Manager will provide Housing Stabilization Services and will  be responsible for engaging with a caseload on site at Indoor Villages to provide support  in maintaining housing, and assist them with planning for, finding and moving to a home in the community.   Program staff utilize Harm Reduction and Housing First models to assist individuals who have experienced long-term homelessness to find and maintain housing. 

Avivo provides a continuum of integrated services, including access, clinical treatment, care coordination, supportive housing, and recovery support.  We provide services that are innovative, person centered, strength based, and outcome driven.  We strive for the highest quality of services using culturally responsive harm reduction in a trauma informed environment.  

 

ESSENTIAL RESPONSIBILITIES:

  • Provide flexible services within a schedule that meets the needs of participants and offer regular and quality contact to build relationships and maintain trust.
  • Develop individual service plans and housing transition plans with participants and provide supports necessary to achieve identified goals. 
  • Perform general case management tasks as needed including assistance to apply for benefits, increasing various skill levels, working towards increasing financial independence, connecting with mental/chemical/medical health providers, and promote self-determination.
  • Engage in all aspects of Rule 79 Adult Mental Health Targeted Case Management and Housing Stabilization Services.
  • Develop a crisis plan and conduct crisis intervention as needed to ensure holistic health and well-being as well as housing stability.
  • Communicate with case management team members, community professionals, and other service providers to ensure that services for clients are coordinated.
  • Maintain client files, program records, court reports, and complete reporting and documentation in a timely manner, as required.
  • Participate in staff meetings, clinical supervision, and meet regularly with program coordinator for individual supervision, fulfilling supervision requirements.
  • Comply with HIPPA and Data Privacy Practices.
  • Positively represent agency, program, and participant needs.
  • Actively participate in agency’s undoing racism plan.
  • Assist in direct supervision of clients during open social / recreation activities.
QUALIFICATIONS:
  • Associates Degree and 1,000+ hours of experience working with adults that are diagnosed with dual disorders.  Bachelor’s degree with experience is preferred.
  • Experience working with single adults who have dual diagnosis, that have episodes of housing instability and homelessness. Knowledge of local housing resources and systems.
  • Experience with and/or commitment to working in a diverse, continuum of services in a fast-paced, growing organization along with a demonstrated commitment to developing and delivering culturally responsive services.
  • Must successfully pass the online Minnesota DHS Housing Sustaining/Transition Training.
  • If eligible to be licensed, applicant must maintain licensure and communicate any changes in licensure status to supervisor immediately.
  • Previous work experience in a behavioral health setting, and with programs serving individuals with dual diagnoses helpful.
  • Must be available to work outside regular business hours on an occasional basis to meet client and program needs. 

 

SALARY:  $19.45 - $24.31 per hour,  DOQ
