The Housing Stabilization Services Case Manager Float (HSS Case Manager) will be working specifically with individuals that are residing in Avivo’s Transitional Recovery Housing.  The HSS case manager serves single adults who are attending Intensive Outpatient Treatment for Dual Disorder Treatment. The HSS Case Manager will be responsible for responding to housing crisis and client concerns, supporting participants to maintain their housing, assist them with their housing transition plan, and meet their individual service plan goals. They will also be responsible for housing searches and landlord relations. They will do this in coordination with the other HSS Case Managers, the Targeted Case Manager, LADC counselors, therapists, and the Clinical Manager. This position requires a High School Diploma.



  • Demonstrate commitment to SAMSHA’s definition of recovery and recovery principles: “A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.”
  • Actively participate in the division’s Trauma Informed Care Initiative and motivational interviewing.  Support and implement evidence-based practices related to specific program area.
  • Provide flexible services within a schedule that meets the needs of participants and offer regular and quality contact to build relationships and maintain trust.
  • Respond to client crisis as they arise and work to problem solve and deescalate situations.
  • Develop individual service plans and housing transition plans with participants and provide supports necessary to achieve identified goals. 
  • Provide general case management tasks for anything pertaining to housing including aiding in applying for benefits, increasing financial independence, increasing various skills, connecting with mental/chemical/mental health providers, and promising self-determination.
  • Engage in all aspects of Housing Stabilization Services.
  • Develop a plan and conduct interventions as needed to ensure housing stability and holistic health and well-being.
  • Communicate with case management team members, community professionals, and other service providers to ensure that services for clients are coordinated.
  • Maintain client files and program records, court reports, and complete reports/documentation as required.
  • Participate in staff meetings and meet regularly with program coordinator for individual supervision to ensure supportive client care.
  • Comply with HIPPA and Data Privacy Practices.
  • Continuously exercises good judgment in handling participant issues and represents organization with a positive and professional image. At all times projects a favorable image of organization to promote its aims and objectives.
  • Provides and promotes unconditional positive regard to all participants regardless of race, creed, gender, religion, sexual orientation, disability, and economic status.
  • Promote and support involvement in the organization’s commitment to undoing racism and reducing disparities.
  • Performs other duties as requested or assigned.
  • Meets performance criteria as outlined in annual performance evaluation and individualized performance standards.
  • This position will require the ability to work outside regular business hours on a regular basis to meet client and program needs. 



  • Housing Stabilization Case Manager position requires a High School Diploma.
  • Experience with single adults who have experienced long-term homelessness/with the target population served, and knowledge of housing resources in the community.
  • Ability to work during and outside of normal business hours to meet the needs of the participants.
  • Demonstrated understanding of the recovery philosophy and recovery oriented behavioral health services, or willingness to learn recovery principles.
  • Experience with and/or commitment to working in a diverse, continuum of services in a fast-paced, growing organization.
  • Experience with and/or a demonstrated commitment to developing and delivering culturally responsive services.
  • Ability to de-escalate and mediate client situations.
  • Able to work independently with participants in their home rooms.

WAGES:  $19.45 -$24.31 per hour, DOQ
