Description

Summary:

  • Under the direction of the Residential Staff Supervisor, the Case Manager is responsible for (1) providing crisis intervention, advocacy, information and referrals for individuals calling the agency's 24-hour hotline; and (2) accessing the needs of residential clients and implementing case plans for individuals and/or families whose lives have been impacted by relationship violence. The Case Manager is responsible for providing supportive services to clients that focus on securing permanent housing, employment, and childcare while utilizing best practices of trauma informed care. The Case Manager will work closely with community partners to ensure clients have access to resources and services that will further support self-sufficiency, safety, and housing security.

 

Essential Duties and Responsibilities:

 

  • Assess financial, social and emotional needs of client; utilize a strength based approach when problem solving
  • Work with clients to create a case plan that focuses on short and long term goals; meet with clients as needed to review, access, and support progress
  • Advocate for the client, as needed, to community partners, programs, and within the agency
  • Collaborate with community partners, social workers, and other service providers to secure community resources for client
  • Serve as a support to families in crisis; complete reassessment of needs, as appropriate
  • Respond to 24-hour hotline for callers and survivors needing support due to relationship and family violence; provide crisis intervention counseling, advocacy, information and referral as appropriate.
  • Oversee hotline office and necessary emergency shelter activities throughout assigned shift. The advocate may be the only staff member present on-site during an evening, overnight, or weekend shift, and will be solely responsible for coordinating all shelter activities; an on-call supervisor will be available for crisis support and safety matters for all residential facilities. 
  • Conduct screenings for prospective clients and arrange emergency hotel stays, as appropriate
  • Arrange emergency transportation for clients, as appropriate
  • Complete intake paperwork with new shelter clients
  • Conduct facility checks throughout shift to ensure safe and satisfactory operations, assess for maintenance needs; be available to assist clients, as needed
  • Develop and maintain relationships with community partners
  • Attend all appropriate collaborative meetings
  • Provide follow up support services to former clients, as appropriate
  • Facilitate psychoeducational groups as requested by supervisor
  • Submit paperwork that successfully meets agency guidelines
  • Collect and input data into corresponding database as needed for Human Options and relative grants
  • Participate in supervision meetings with Supervisor
  • Attend weekly and all Human Options staff meetings
  • May be required to work evenings and weekends, as needed
  • Provide support for Human Options staff ,as needed
  • Demonstrate ability to abide by the values of Human Options: show dignity, respect, honesty, integrity, and a commitment to excellence and maintain a supportive environment
  • Abide by professional confidentiality guidelines by only sharing client information within the organization when it meets the need of the family
  • Report all suspected child, elder, dependent adult abuse to the appropriate agency as mandated by law
  • Act as a community ambassador for Human Options and relationship violence awareness
  • Other duties as assigned
Qualifications

 

  • English fluency required, Bilingual in Spanish or Farsi preferred
  • Bachelor’s degree in human services or related field required
  • Must have experience working within nonprofit agencies
  • Excellent verbal and written communication skills
  • Knowledge of the child welfare system
  • Must have familiarity with and willingness to utilize community resources in Orange County
  • Two (2) years’ experience in case management with families is preferred
  • One (1) year of experience working with at risk children and families is preferred
  • Computer literate at the intermediate level (i.e. Microsoft Word, Outlook, and Excel)
  • Valid CA Class C license and auto insurance, with ability to travel to meetings, trainings and home visits as needed

ADA Job Characteristics:

Work environment is primarily in an emergency 24-7 shelter facility setting.  Works in a sometimes fast paced setting dealing with crisis intervention.  May on occasion need to go up one flight of stairs to meet with clients.  Lift and carry up to 25 pounds up to waist length, extend both arms above the head and/or reach below the waist, stoop, squat, crawl, bend the back to open lower-level filing drawers, or to retrieve stored items, climb on stools, steps and/or ladders occasionally.  Sit and/or stand frequently.  Use fine manipulation (hands and wrist) to operate a keyboard, take notes, complete and file reports, file and copy documents.  Corrected vision to normal range.  Speak in normal vocal tone and range, and communicate clearly and concisely to and with others.  Apply normal reasoning and detail as required in the accomplishment of job duties.
