The Chemical Health Assessor will conduct chemical health assessments with individuals accessing care at Avivo and facilitate admission to 245G programs. This role will ensure that all individuals receive access to appropriate services and assistance in gaining access services. This role will work in a dual capacity between the 1900 campus, Avivo Village (as well as other internal programs), and other community partners to increase client opportunities to direct access. This individual will work as part of the Avivo Access Team.  Schedule: Monday –Friday (7:00am-3:30pm), with the ability to work a flexible schedule.

We provide family residential treatment, co-occurring outpatient treatment for individuals and families, chemical use assessments,peer recovery support,mental health case management services, care coordination,and Community Support Programming. Services are innovative, person-centered, strength-based, and outcome-driven.   • Conduct chemical health assessments, ensuring that assessments are completed in a timely manner and that consumers receive assistance accessing recommended services. Assessments are conducted at Avivo and other locations. 
  • Screen individuals referred to Avivo to establish appropriateness and/or eligibility for program. 
  • Conduct 245G intake and orientation and ensure compliance with 245G and other program expectations. 
  • Follow up to ensure that central access functions are performed and completed in a timely manner. 
  • Ensure service coordination for care and services within Avivo and community-based agencies. 
  • Provide services that meet professional and ethical standards. 
  • Actively participate in Avivo’s trauma informed care initiative. 
  • Responsible for learning, understanding & promoting Avivo services. 
  • Provide public relations and outreach to various community agencies. 
  • Communicate effectively with funding authorities and treatment providers, ensuring that Avivo programs are included.  
  • Utilize electronic health records and keep all documentation up to date. 
  • Support Avivo’s mission and maintain production expectations. 
  • Attend meetings, supervision, and trainings as required. 
  • Participate in effective communication and accept supervision with/from supervisor
  • Manage miscellaneous projects and complete various tasks, as assigned by supervisor. 

We REQUIRE a Chemical Health Professional that is either:

  1. Licensed Alcohol and Drug Counselor (LADC), or
  2. Dually Licensed: Licensed Alcohol and Drug Counselor (LADC) and Licensed Mental Health Professional (LICSW, LP, LMFT, and LPCC).

Must be licensed and in compliance with the appropriate licensure board. (i.e. Board of Behavioral Health & Therapy, Board of Social Work, Board of Marriage and Family Therapy, or another licensing board.)

Additional Requirements:

  • Two or more years of experience in a Behavioral Health setting. 
  • Training and experience in Rule 25 Assessment and 245G Comprehensive Assessment.  Preference given to candidates with approved Hennepin County Assessor training. 
  • Experience with an electronic health record required: knowledge of Electronic Medical Records preferred.

  • Training and/or experience in trauma informed care. 
  • Experience working with individuals from diverse backgrounds.  
  • Ability to work as a team member with coworkers and referral sources.   


WAGES:  $22.47 - $28.09, DOQ
