The Avivo treatment team provides co-occurring chemical and mental health treatment, including individual and group therapy and diagnostic assessment.  Our model is innovative and focused on the vast and very complex needs of our community. We utilize trauma-informed, evidence-based, and culturally competent treatment strategies to engage participants.  Strong emphasis is placed on delivery of clinical services through multidisciplinary teamwork and coordination. This position provides services to individuals participating in our 245G co-occurring treatment outpatient services, working with men, women, and women who are parenting.

Avivo's Chemical & Mental Health Services division provides integrated services in the areas of substance use disorder treatment and community-based chemical and mental health. We provide family residential treatment, co-occurring outpatient treatment for individuals and families, chemical use assessments, peer recovery support, mental health case management services, care coordination, and Community Support Programming. Services are innovative, person-centered, strength-based, and outcome-driven. We strive for the highest quality of services using culturally responsive and evidence-based practices in a trauma-informed environment.



  • Complete thorough diagnostic assessments during intake process.
  • Provide clinical services through therapy techniques that meet state, professional and ethical standards for 245G and Rule 29 services.  Provide both individual and group services.
  • Upon assessment, ensure individuals in need of medical care, psychiatric care and/or case management receive timely access to needed resources and services (internally or externally).
  • Collaborate with chemical health professionals to ensure comprehensive care.
  • Assess individual’s stability and level of service need in the recovery process. Assess individual needs and make appropriate referral and service coordination for community-based services.
  • Work with individuals to enhance stability according to evidence-based practices.
  • Participate in team meetings, peer reviews, treatment plan development, treatment plan review, and clinical consultation/ supervision.
  • Coordinate care with internal and external providers on a regular basis.
  • Maintain accurate and timely records according to Rule 29, 245G and other state guidelines for purposes of effective client interventions, accountability, and administration.
  • Pursue and maintain credentialing with third party payers to delivery therapy services.
  • Complete necessary paperwork for billing, statistical/outcome reporting, and prior authorizations.
  • Facilitate mental health training for staff as requested or assigned.
  • Complete documentation in Procentive electronic health record.
  • In an effort to support the health and well-being of our staff, participants, and the community,  Avivo will require proof of COVID Vaccination for all New Hires.
  • Licensed Mental Health Professional (LICSW, LMFT, LP or LPCC). Must be licensed and in compliance with the appropriate licensure board, i.e. Board of Behavioral Health & Therapy, Board of Social Work, Board of Marriage and Family Therapy, or another licensing board.  
  • Dually licensed professional in mental health and chemical health (LADC) desired.
  • Experience and competency in working with individuals and families from diverse backgrounds with very complex needs.
  • Ability to utilize broad-based knowledge of therapeutic and evidence-based interventions for adults, children and families.
  • Demonstrated experience providing diagnostic assessments.
  • Must have training and experience in co-occurring treatment (substance use disorder and mental health).
  • Current credentialing with third party payers desired.
  • Competence in trauma-informed care and evidence-based practices desired.
  • Experience providing coordination of services with community mental, physical, and chemical health providers and other key stakeholders desired.
  • Experience with electronic health records systems desired (Procentive).
  • Experience with or knowledge of the collaborative documentation service model desired.
  • Ability to pass/ clear DHS Background Study.
  • 2+ years freedom from problematic substance use.

SALARY:  $54,015 - $67,519 / year, DOQ
