Join a dynamic team of case managers providing services for individuals receiving Rule 79 Adult Mental Health Case Management.  The Mental Health Case Manager will provide Rule 79 Targeted Case Management services to adults with mental illness to assist them to live successfully in their goal living situation.  Avivo staff work to be a part of the solution in improving people’s well-being.  This case management team assists adults with serious mental illness in pursuing an outcome of improved or maintained mental health functioning.  The role will allow the case manager to apply innovative approaches in the provision of supportive mental health services.

Avivo's Chemical and Mental Health division provides integrated services in the areas of substance use disorder treatment and community-based chemical and mental health. Services are innovative, person-centered, strength-based, and outcome-driven. We strive for the highest quality of services using culturally responsive and evidence-based practices in a trauma-informed environment. We provide family residential treatment, co-occurring outpatient treatment for individuals and families, chemical use assessments, peer recovery support, mental health case management services, care coordination, and Community Support Programming.

ESSENTIAL RESPONSIBILITIES:

• Assist clients in finding a safe, affordable placement; providing necessary support services and assuring that any required additional services and providers are informed and in place. 
• Conduct an initial assessment and develop an Individual Community Support Plan that will assist clients to address their needs, deficits, and goals.   
• Refer clients to appropriate community resources and arrange transportation to appointments as needed. 
• Participate in clinical supervision, staff meetings, and meet regularly with program coordinator for individual supervision to meet supervision requirements. 
• Comply with HIPPA and Data Privacy Practices. 
• Visit clients in their home as well as the community on a regular basis to check progress, provide support and monitor mental health functioning and medication effectiveness. 
• Ensure timely completion of required court documentation for civil commitment proceedings.   
• Communicate with case management team members, community professionals, and other service providers to ensure that services for client are coordinated. 
• Maintain client files and program records, court reports, and develop reports as required. 
• Positively represent agency, program and participant needs. 
• Actively participate in agency’s undoing racism plan. 
• This position may require the ability to work outside regular business hours on an occasional basis to meet client and program needs. 

• Bachelor’s Degree in Behavioral Sciences or related fields including but not limited to social work or psychology from an accredited college or university or Associate Degree with four years of experience as case manager.  Master’s degree preferred. 
• Must have at least 2,000 hours of supervised experience in the delivery of services to persons with serious mental illness. 
• Working knowledge of harm reduction and housing first. 
• Experience working with a diverse population of people, delivering culturally responsive services.
• Experience working with individuals experiencing homelessness. 
• Experience working with a caseload is preferred. 
• Ability to relate to and advocate for diverse client needs and interests. 
• Commitment to completing paperwork accurately and in a timely manner. 
• Able to learn and understand client needs and mental health recovery related to the provision of community mental health services. 
• Strong written and verbal communication skills and computer experience.

WAGES:  $19.45 - $24.31, DOQ
