Developmental Pathways is seeking a Community Outreach Coordinator who will coordinate support services for adults and/or children living in a variety of settings. Our Coordinator will communicate with a broad team of providers including professionals, direct care professional employees, families, and natural support chosen by the individual or family being served. You’ll also get to assist with our department’s initiatives and outreach efforts.

Additional Responsibilities
  • Research, request, and obtain potential services and/or providers to meet individual needs/preferences as determined in the Community Outreach Enrollment Form
  • Mentor individual’s service choices; help families determine priorities for service
  • Respond promptly and professionally to individual/family needs and requests
  • Facilitate referrals to other agencies as required
  • Actively develop collaborative relationships with providers and other community partners
  • Prepare family for transitions to other services as appropriate
  • Attend and participate in meetings; schedule and facilitate as needed
  • Ensure all Federal and State rules and regulations, and agency policies and procedures are adhered to in the provision of services
  • Complete Allocation Requests and Check Requests for individual/family needs
  • Work with services providers and independent contractors to assure expenditures are within budgetary guidelines
  • Develop and maintain case files

Starting Salary and Benefits
$21.90- $23.54 hour or $22.95 - $24.59 hour for Bilingual Coordinators

  • Associate Degree in human services related field
    • OR 5 years with a combination of education and experience appropriate to the requirements of the position
  • Excellent oral and written communication skills; can effectively communicate with individuals served, families, staff, and others
  • High degree of accuracy with time-sensitive, detailed information
  • Ability to meet and maintain our driving requirements; no suspensions and less than three (3) tickets in three years
  • Personal vehicle in good operating condition for use during work as appropriate
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
  • Working knowledge of the DD system, rules, and regulations
  • Two (2) years of experience in service coordination/case management in a similar agency
Additional Requirements for Bilingual Coordinators
  • Able to speak, write and interpret fluently in English and Spanish
  • Ability to complete all typical case management responsibilities in Spanish
  • Support with written translation of internal documents for use by stakeholders, as appropriate
