The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

Since January 2020, DOHMH has been fighting the COVID-19 pandemic- activities include but are not limited to: surveillance & epidemiological investigations; lab testing; engaging and providing guidance to healthcare partners; ensuring the safety & health of responders; providing timely and accurate information to the public; promoting the emotional health of New Yorkers and coordinating citywide COVID-19 vaccine administration. DOHMH works closely with community-based organizations (CBOs) to increase access to and awareness of COVID-19 prevention, treatment, and vaccination services. Consistent with the Health Department’s decades long commitment to place-based investments, new work is focused on neighborhoods who have been hardest hit by the COVID-19 pandemic, have low vaccination rates, and have been harmed by racial and ethnic health inequities. DOHMH seeks to continue place-based anti-racism interventions toward COVID-19 recovery and chronic disease prevention over the coming two years through continued investment in the Public Health Corps. 



DOHMH is seeking a Community Health Worker to align multiple streams of COVID-19 prevention and recovery and chronic disease prevention work until June 2024  


This role sits in the Bureau of Bronx Neighborhood Health in the Center for Health Equity & Community Wellness (CHECW), which seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW’s work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers. CHECW is a division of the NYC Department of Health and Mental Hygiene. 


The Bureau of Bronx Neighborhood Health is seeking to hire a Community Health Worker who will be responsible to promote COVID-19 recovery efforts and to improve the health status of neighborhoods by engaging the community, conducting outreach (e.g., neighborhood canvassing) and promoting resources, services, and programs for health and wellness. The Community Health Worker under general supervision from the Director of Community Engagement, with latitude to serve as a team lead performs responsible work in any phase of community programs, engagement, and outreach. This includes providing community engagement and programmatic support in promoting health equity and COVID-19 recovery throughout the Bronx.  



  • Assist with planning and executing health awareness and promotion activities in diverse settings (some evening and weekends work required)
  • Engage community partners and local organizations, on health information and education resources to improve health and/or COVID-19 recovery
  • Conduct outreach, develop relationships, and strengthen collaboration with community residents, organizations, and key stakeholders program partners to improve health equity in the Bronx
  •  Input community outreach, detailing and programmatic efforts in database platforms
  • Assist with program administration for community outreach and detailing work and provide clerical support as needed
  • Provide support in the emergency response efforts including but not limited to disseminating COVID-19 and Bronx Bureau programmatic information and resources (e.g., PPE) to increase access to and awareness of services
  • Assist with managing, monitoring, and tracking all COVID-19 and programmatic resource distributed to the public
  • Support other programmatic needs within the Bureau   
  • Other duties as assigned by supervisor 
  • A Baccalaureate degree from an accredited college and 3 years of full-time satisfactory experience in community work
  • Quick learner and self-starter 
  • Works well independently and within team  
  • Some community engagement experience, a plus 
  • Familiarity with data entry and tracking  
  • Excellent verbal and written skills; presentation skills a plus 
  • Experience working and/or engaging in priority neighborhoods 
  • Strong interpersonal skills and the ability to build trust and maintain relationships with community partners 
  • Strong organizational skills with good follow-through to complete assignments within specified timeframes 



  • Bilingual in Spanish or French  
  • Demonstrable experience in addressing drivers of COVID-19 vaccine hesitancy and chronic disease 
  • Ability to understand the concepts of social determinants of health, institutional racism, structural racism, health inequities and their impact on underserved and underrepresented communities 
  • Demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g., people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)

35 hours per week. This position will be funded through June 30, 2024, with possibility of extension. 



FPHNYC offers a comprehensive benefits package. The salary for this position is $60,000 a year.



There is potential for this position to transition to DOHMH and therefore candidates must meet DOHMH eligibility requirement including NYC residency.  All hires must be vaccinated against COVID-19 unless they have been granted a medical or religious reasonable accommodation. This requirement must be met by your hire date.  This position will operate in a hybrid work environment with the potential of shifting to 100% in-office.
