Avivo is offering a $2,000  hiring bonus for this position. The Case Manager will be working specifically with individuals experiencing long term homelessness, exiting out of treatment, currently or formerly experiencing unsheltered homelessness, and affiliated with Red Lake Nation through the Housing Support Hennepin program. The Case Manager with the Housing Supports Program (Formerly Group Residential Housing – GRH) serves single adults affiliated with Red Lake Nation, experiencing homelessness, in scattered site apartments in the community.  The Case Manager will be responsible for housing search and landlord relations, as well as rule 79 Adult Mental Health Targeted Case Management and Housing Support program management.  This program utilizes Housing First and Harm Reduction models to assist individuals who have experienced long-term homelessness to find and maintain housing.    


  • Ensure high quality, mission-driven services   
  • Demonstrate commitment to SAMSHA’s definition of recovery and recovery principles: “A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.” 
  • Engage in all aspects of Rule 79 Adult Mental Health Targeted Case Management and Housing Support Case Management utilizing both assertive and progressive engagement modalities. 
  • Creatively reduce harm caused by drug and alcohol use through a continuum of approaches and in collaboration with team members and community partners. 
  • Problem-solving a range of barriers to housing stability and connection to supportive mental health and substance use disorder services – being creative and “think outside of the box”.  
  • Employ a holistic and culturally-appropriate approach when interacting with residents including promoting health and wellness activities with participants.  
  • Utilize Critical Time Intervention to support clients work towards their goals by creatively strengthening their network of support in the community. 
  • Conduct an initial assessment and develop an Individual Community Support Plan, along with a Functional Assessment, which will assist clients to address their needs, deficits, and goals. Ensure ongoing bi-annual assessments are timely completed.
Education & Experience


  • BA in Behavioral Science with a minimum of 2000 hours of employment in Mental Health, working with adults with Serious and Persistent Mental Health Diagnoses.  Master’s Degree preferred.  
  • Experience: 2 years’ experience working with single adults who have experienced long-term homelessness.  
  • Experience: 2 years’ experience working with individuals experiencing substance use disorders.  
  • Experience: Minimum one years’ experience working in Harm Reduction and Housing First with a demonstrated working knowledge and commitment to these modalities.  
  • Experience working with a caseload is preferred. Broad knowledge of case management activities and wrap-around services.
