Intro/Program Description: Capitol Hall is a permanent supportive housing residence for formerly homeless adults. On-site services include case management, entitlement assistance, medical and mental health services, substance abuse counseling, work readiness training, linkages and referrals, a meals program, 24 hour/7 day per week front desk security, and daily group activities.

Purpose of Position: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults with a history of chronic illness, mental illness and/or substance use disorder with the goal of maintaining permanent housing.

Schedule: Monday through Friday - 9:00am to 5:00pm

Salary: $36,076 - $45,095

Client service

• Provide client-centered case management services to formerly homeless tenants with medical and behavioral health needs.
• Identify short and long-term treatment goals and work with clients to achieve them, providing linkage to community programs such as medical, psychiatric and substance abuse treatment.
• Maintain timely case records with documentation of services provided. Track tenant data and outcomes in accordance with specific program procedures.
• Assist clients in receiving entitlements and in understanding and complying with building rules and government subsidies to maintain residency.
• Coordinate and follow up with the onsite and off-site medical and mental health treatment providers.
• Accompany clients on visits to doctors, entitlement offices or other community services.
• Collaborate with staff in other GRCC programs and community providers to coordinate client service.
• Participate in the clinical integration project to identify and address needs of most vulnerable tenants
• Use data as a tool to make decisions on service deliverables and best practices for a results orientated mind set
• Develop and facilitate group activities geared at increasing socialization and reducing isolation.

Participate as Capitol Hall team member

• Attend staff meetings, case conferences, professional development trainings and supervisory meetings.
• Collaborate and consult with onsite medical personnel to address tenant needs.
• Participate in planning and leading group activities.
• Other duties as required include conducting room inspections, incident reporting and crisis intervention.

Qualifications/Educational Requirements:

• Bachelor’s Degree Required
• Professional or academic knowledge of working with older adults with mental illness, substance use disorder and adults experiencing homelessness a strong plus.
• Spanish speaking preferred

Computer Skills Needed

• Proficient in Microsoft Outlook and Excel
• Ability to use or learn computer databases such as AWARDS

Physical Requirements

• Some walking and travel by public transportation to escort clients to various appointments throughout the city.
• Occasional visits to outside service providers, such as rehab centers, hospitals and medical clinics.

Work Environment

• Mostly indoor office, occasional kitchen and basement setting during group activities and events.
• Regular home visits to client apartments to assess safety is required.
