The Case Manager provides Rule 79 Case Management services to adults with mental illness, with an utter commitment to help them live successfully in their goal living situation. This position also assists adults with serious mental illness in pursuing the outcome of improved or maintained mental health functioning. This is accomplished by finding a safe, affordable placement; providing necessary support services; and assuring that any required additional services and providers are informed and in place.

PRIMARY RESPONSIBILITIES:

• Work with Hennepin County and meet with clients face to face, over the phone, or virtual.
• Go to the clients’ homes, meet at places in the community, or take the clients to appointments. 
• Typical case load is around 28 clients.
• Clients are mid-range intensity; the acuity is severe and persistent mental illness.
• Carries 4-5 commitment clients per caseload, most court appointments are on zoom.

• Bachelor’s degree in one of the behavioral sciences or related fields + 2,000 hrs. supervised experience with individual with serious mental illness/substance abuse
• Ability to work outside of normal business hours to meet the needs of the participants
• Proof of COVID-19 vaccination
