Summary

SUMMARY:

Avivo is offering a $2,000  hiring bonus for this position. The Mental Health Case Manager in the Intensive Case Management Program will provide Rule 79 Case Management services to adults with mental illness, to assist them to live successfully in their goal living situation. This is accomplished by finding a safe, affordable placement; providing necessary support services; and assuring that any required additional services and providers are informed and in place. This position also assists adults with serious mental illness in pursuing the outcome of improved or maintained mental health functioning.     

PRIMARY RESPONSIBILITIES:

Ensure high quality, mission-driven services   

  • Demonstrate commitment to SAMSHA’s definition of recovery and recovery principles: “A process of change through which individuals improve their health and wellness, live a self-directed life, and strive to reach their full potential.” www.samhsa.gov 
  • Conduct initial assessment and develop an Individual Recovery and Community Support Plan (ICSP) that will assist clients to address their needs, deficits, and goals.  The plan of service is developed with the involvement of the client, family, or other persons of significance to the client.   
  • Evaluate, change, and further specify recovery goals on a continuing basis with each client.  
  • Visit clients on a regular basis to check progress, provide support and monitor mental health functioning and medication compliance.  
  • Provide appropriate referrals to community Avivos, and arrange transportation to appointments as needed.   
  • Communicate with other community professionals and providers to ensure that services for clients are coordinated. 
  • Maintain client files and program records, court reports and develop reports and billing statements as required.  
Education & Experience

Required:

  • Education: Bachelor’s degree in one of the behavioral sciences or related fields, including but not limited to social work or psychology from an accredited college or University.  

  • Experience:  have at least 2,000 hours (about 2 and a half months) of supervised experience in the delivery of services to individuals with serious mental illness, and/or persons with mental illness and substance abuse.  

  • Licensure: If licensed or license eligible, must maintain valid licensure and communicate any changes in licensure status to supervisor immediately.   

  • Ability to work outside of normal business hours to meet the needs of the participants.  

  • Demonstrated understanding of the recovery philosophy and recovery oriented behavioral health services, or willingness to learn recovery principles. 

  • Experience with and/or commitment to working in a diverse, continuum of services in a fast-paced, growing organization. 

  • Experience with and/or a demonstrated commitment to developing and delivering culturally responsive services. 

  • Avivo requires all new hires to be Covid-19 Vaccinated upon start date.
