Education:
- Bachelor's Degree in Social Work, Human Services, or related field.
License and/or Certification:
- Certification in Case Management, Psychology, or Social Services preferred.
Experience:
- Three plus years working with individuals with disabilities.
- Three plus years case management experience.

- Proficient in Windows, including the Microsoft Office business application suite; and data management systems.
- Ability to perform detailed work with numerical data and to make arithmetic computations accurately.
- Capable of working with a variety of personalities and leadership styles.
- Detail-oriented and highly organized; capable of effectively balancing multiple priorities.
- Flexible with workload and schedule to meet demands of the job.
Physical Demands:
- Ability to remain in a normal seated position regularly.
- Ability to move and lift items usually ranging up to 25 lbs.
- Occasional travel is required.
Requirements:
- Must possess reliable transportation.
- Must pass Level II background check.
