Bournewood Health Systems is  seeking a Social Worker/Clinician for an Adult Partial Hospital Program.  The incumbent’s prime responsibilities will be to provide direct clinical service to assigned Bournewood Health Systems Partial Hospital patients, inclusive of individual and family treatment interventions, case management, disposition planning, care consultation and utilization review. Is responsible for initial assessments for each patient and treatment planning. Meets with other disciplines regarding patients treatment plan. Consults on an as needed basis with other team members around issues of his/her expertise, including case management and disposition planning. Leads psychotherapy and/or psycho-educational groups for Partial Hospital patients.

Qualifications:  Candidates must have strong organizational and interpersonal skills, and the ability to multitask. Partial Hospital or Inpatient experience with adults is strongly preferred.  Social worker requirements include the completion of a Masters Degree in Social Work from an accredited school of Social Work or Masters in Counseling and Licensed eligible (LMHC).

Essential Functions:

1. Leads daily group therapy sessions as indicated by Supervisor.
2. Assumes role of social worker for assigned patients.
3. Meets with patients and guardians to complete the written initial assessment within 24 hours of patient admission, and completes treatment plan within 48 hours.
4. Provides and maintains contact/consultation, (as patient’s guardian permits) with collateral supports including but not limited to patient’s outpatient therapist, prescriber, PCP, school, DCF/DMH case worker.
5. Involves and updates families on treatment planning, treatment progress, aftercare planning and educates family on available resources.
6. Coordinates/organizes meeting of outpatient providers or school personnel as necessary.
7. Communicates findings and progress of group, individual, and family therapy and discharge recommendations/planning to treatment team.
8. Consults on an as needed basis with other team members around issues of his/her expertise.
9. Maintains daily progress notes in charts and completes group notes for all groups performed.
10. Ensures appropriate case management and aftercare appointments with providers and informs patient and family of all appointments.
11. Conducts utilization review on cases assigned by the Supervisor.
12. Documents all clinical and collateral contacts and case management activities in a timely and professional manner.
13. Complies with all statues and regulations relevant to the maintenance of clinical standards in the Commonwealth, e.g. mandated reporting, duty to warn, confidentiality, human rights.
14. Complies with the policies and procedures of Bournewood Health Systems.
15. Adheres to the Social Work Code of Ethics.
16. Performs other related duties as requested by the Supervisor.

Essential Competencies:

1. Knowledge of major mental illness, i.e. schizophrenia and the impact of family.
2. Knowledge of developmental stages and clinical implications regarding abandonment.
3. Knowledge and understanding of biological and sociocultural changes influencing adolescents, adults, and geriatric populations.
4. Knowledge of substance use disorders and associated treatments.
5. Knowledge of family systems dynamics.
6. Knowledge of group therapy and group dynamics.
7. Ability to conduct individual assessments and provide effective clinical documentation.
8. Is competent in delivering patient care that is trauma informed and patient centered. As indicated by:
   1. Provides care that is kind, collaborative and compassionate.
   2. Able to defuse power struggles and doesn’t need “to win”.
   3. Appropriate assessment of clients for a history of trauma
   4. Recognize/assess a range of trauma-related behaviors
   5. Implement trauma-sensitive interventions with people served

Minimum Qualifications:

Social worker requirements include the completion of a Masters Degree in social work from an accredited school of social work. 

Physical Criteria:

1. Must be able to climb stairs and walk between buildings.
2. Must be able to use a telephone and computer.
3. Must be to sit for up to 8 hours.
4. Must be able to provide crisis intervention techniques when necessary.
