Recovery Network of Programs, Inc. (RNP), is a private, non-profit, social service agency which has been serving the Greater Bridgeport Community and Fairfield County area since 1972. RNP is dedicated to helping those living with substance use, mental health disorders, and homelessness. Iranistan House - Transitional Living Center The Transitional Living Center is an 8-bed group home residential facility for men and women who suffer from severe and persistent psychiatric disabilities. The program provides residential and rehabilitative services to individuals who have significant skill deficits in the areas of self-care and independent living and who require a non-hospital, supervised community-based residence. Staff strives to protect the privacy and personal dignity of each individual while teaching, coaching, educating, training, counseling and assisting with daily living and self-care skills to enable each client to assume increased responsibility necessary for a full and independent life in the community.
Hours: Monday - Friday 8am - 4pm
CORE FUNCTIONS
- Provide and document individual, family, and group counseling directed at solving daily problems related to community living and interpersonal relationships.
- Facilitate or co-facilitate psycho-educational groups about the alleviation and management of psychiatric and/or substance abuse disorders and document progress.
- Assess and document significant medical, psychological, and social factors contributing to a person served and inform other service providers involved promptly to coordinate the appropriate services.
- Record adjustments and progress concerning finances, employment, food, clothing, housing, physical and mental health well-being.
- Acknowledge and document all behavior(s) on the part of the persons served that is considered inappropriate or a cause for concern.
- Attempt to redirect any person respectfully and appropriately, offer supportive counseling and utilize crisis intervention techniques & contingency plans whenever necessary based on the need and staff assessment.
- Record and monitor individual rehabilitation/recovery plans and perform follow-up to determine quantity and quality of service provided to the persons served.
- Advocate on behalf of persons served in receiving the required services needed to become successful and satisfied in the community.
- Establish and maintain cooperative relationships with community resources, service providers, and other organizations.
- Assume all other appropriate duties as deemed necessary by the supervisor.
