Job Title: Youth & Family Liaison

Reports to: ASPIRE Program Coordinator

Status: Full-Time (32 hours), Exempt

Location: East Side Neighborhood Services Hybrid (in office + community)

East Side Neighborhood Services has an opening for a Youth & Family Liaison, as part of an expansion to our ASPIRE program. This program provides wraparound case management services to youth and families dealing with truancy and contributing factors such as housing insecurity, mental health, or learning challenges. Clients are referred to the program from schools, community partners, and the Hennepin County Attorney’s Office Be@School program. Liaisons works closely with students and caregivers to increase their involvement in school, while providing support, advocacy and referrals to community resources to address unmet needs. Applicants must be able to relate with and provide outreach to all segments of a multi-cultural community, including under-resourced, under-served youth and their families.

Major Job Responsibilities:

You will be responsible for, but not limited to the following functions:

1. Case Management - 40%

  • Establish and maintain effective and professional working relationships with participants throughout their involvement in the program.
  • Conduct regular school and home visits to assess the strengths and needs of children and families.
  • Monitor grades, school attendance and behavior.
  • Maintain a case load of approximately 15-25 youth and families.
  1. Partner with Families and Schools 20%
  • Develop strong working relationships with parents and guardians.
  • Develop strong partnerships with school personnel, including social workers, counselors, teachers and administrators to help support student needs.
  • Work with families to identify and monitor goals to increase school success and self-sufficiency.
  • Support families to understand and navigate the school and county system effectively.

3. Community Collaboration and Referrals - 20%

  • Build an extensive network of supportive services and programs to assist participants in the accomplishment of their goals, including: mental health, housing, healthcare, out-of-school time youth programs, etc.
  • Gain familiarity and linkages with available support at ESNS.

4. Documentation and Record Keeping 15%:

  • Maintain and complete all necessary forms in youth files as outlined by the Be@School program.
  • Keep daily notes and track all activities.
  • Complete regular data entry into the BASIL and Client Track database systems.
  • Prepare reports as required by ESNS and funders.

5. Other 5%:

  • Transport self and participants to program related off-site locations.
  • Attend and contribute to youth department staff meetings.
  • Other responsibilities as assigned by the Program Manager.

Qualifications

Job Requirements and Qualifications:

Education:

  • Bachelor’s Degree in relevant field such as Social Work, Education, Psychology, or Child Development. [Required per state regulations]

Experience:

  • At least two years of related experience providing intake, assessment, and case management services to children and families experiencing systemic adversities.
  • Successful experience providing home and school-based services.
  • Demonstrated ability to work independently in an off-site setting, manage time effectively, maintain service guidelines and utilize supervision and support.

Other Qualifications:

  • Reliable, insured automobile and valid Minnesota driver’s license with safe driving record. [Required]
  • Ability to work across ethnic/racial/economic lines to provide effective communication and establish trust with clients.
  • Ability to work in a collaborative and coordinated effort with other service providers.
  • Ability to self-start and effectively problem solve.
  • Ability and willingness to work variable hours.
  • Ability to set limits on personal and professional boundaries.
  • Ability to communicate effectively in oral and written forms.
  • Familiarity with computer technology and ability to learn computer programs.
  • Handle confidential information appropriately.

Hours: 32 hours/week; daytime and after-school hours, occasional night/weekend hours.

Compensation: Starting $24/hr depending on qualifications & experience.

Hybrid Expectation: 60% in-office (NE Minneapolis), 40% remote
