We are currently seeking per diem Adult Home & Community-Based Service (HCBS) Providers!

This position will provide Tier 1 HARP/ Adult HCBS services with referred clients.  Services are intended to contribute to the reduction of unnecessary hospitalization/ institutional level or care and improved societal integration of those served. Service delivery is designed to be flexible and individualized to meet the client's needs. Work requires over 50% community travel/visits, including to client homes.  Travel requires use of own vehicle in transporting clients.

The focus of services that Adult HCBS Providers will provide is on the promotion of skill development and identification of community supports in order to overcome deficits related to function and motivation.  This will focus on pre-vocational services, psychosocial rehabilitation, family support and training. 

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned as Adult Home & Community-Based Services are dynamic and the environment can be fluid.  Roles and responsibilities are often expanded to meet the needs of the client as well as tap into the skills and talents of the provider. 

  1. Provide designated services with clients in approved home and/ or community based setting(s) as outlined in each client's Plan of Care.
  2. Coordinate with each client's Health Home Care Manager regarding the frequency, intensity, and duration components of each client's goal(s) as well as the established objectives to meet the goal(s). 
  3. Maintain regular communication with each Care Manager regarding client progress with services and services planning.
  4. Complete and submit all required documentation as associated with service delivery and as outlined by each client's Health Home Care Manager, Managed Care Organization (MCO) representatives and/ or agency supervisor(s).
  5. Demonstrate excellent time and task management skills, as well as oral and written communication skills.


Associates degree and two years related experience, or an equivalent combination of education and related experience.
