Position Purpose

This position is an active member of the Central Administrative team. The Case Aide provides support to the coordinators and care managers in completing required administrative processes and other case management tasks as required to meet department needs with efficiency and accuracy. 


Essential Duties

  • Assists coordinators and care managers by gathering/tracking/entering documentation in various data systems
  • Completes required processes within internal and state system related to each specific task as assigned with great attention to detail
  • Reviews submitted documents as necessary and brings critical information to the attention of the coordinator and care manager
  • Assists the coordinators and care managers with monitoring activity related to service utilization tracking and log note entry in state systems
  • Assists with the eligibility process by proactively sending/tracking and following up as needed with County Departments of Human Services
  • Enters log notes into internal data system or a State System and documents case management activities as it relates to each task
  • Files incoming information as necessary; including attaching documents to internal data system
  • Responsible for accurate and timely completion of all forms, reports, and documentation of case management activities.
  • Conveys a positive and professional image of the organization
  • Communicates effectively with team members and cross departmentally by proactively identifying needs and communicating follow up needs
  • Provides follow-up with coordinators and care managers and provider agencies as necessary in a timely and person-centered manner
  • Maintains professional and ethical manner with all interactions and meets performance, quality, customer service, and coordination standards as assigned by the department management team
  • Performs other duties as assigned

  • Demonstrates support for the company’s mission, vision and values 
  • Basic skill in computer software programs including databases and on-line applications
  • Ability to analyze information and use sound judgment, problem solve, and use critical thinking skills when making recommendations or solving problems
  • Excellent written and verbal communication skills, strong organizational and time management skills, and strong interpersonal skills
  • Ability to maintain strict HIPPA confidentiality guidelines
  • Ability to effectively manage multiple priorities and projects
  • Ability to function as a team player and work cross departmentally


Essential Functions

Minimum Qualifications

  • Bachelor’s degree in Human Behavioral Science or related field of study or a combination of education and/or Human Services or waiver services


Preferred Qualifications

  • Previous experience in a Human Services field


Driving Requirements

  • Valid drivers license
  • Proof of motor vehicle insurance
  • Personal vehicle in good operating condition for use during work 
  • No major violations in the past three years. 
  • No more than two moving violations in the past three years 
  • Ability to meet and maintain agency driving requirements and operate agency vehicles
