Position Purpose

This position will provide caseload coverage, assist with crisis work, problem solving and training new hires on specific job duties and shadowing opportunities.


The outcome for this position will help us to achieve our mission, by simplifying access and developing a person-centered plan that works for each individual and family in creating their future.  These tasks will help with the overall goals for the pilot. 1) assist in employee retention and satisfaction 2) ensure the experience of an individual and/or family is consistently respectful, timely, and focused on whole person 3) a clear understanding of options to individuals and families 4) employees are following outlined process 5) coverage is adequately provided when an employee is out.




Essential Duties

  • Team Lead Duties:
  • Organize training on Desk Level Procedures for new hires and provide job shadowing opportunities
  • Serve as a mentor to staff by  training, guiding and providing ongoing support to new and existing staff
  • Assist with any crisis or complex needs situations
  • Provide coverage as needed for vacancies
  • Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the client and external providers of client services.
  • Educate clients regarding various state plan benefits, programs, options and services.
  • Maintain knowledge of regulations, policies, and procedures regarding current public assistance programs.
  • Responsible for accurate and timely completion of all forms, reports, and documentation of referral coordinator activities.
  • Maintain professional and ethical manner with all interactions and meets performance, quality, customer service, and coordination standards as assigned by the department management team.
  • Participate in training and staff development opportunities. Actively participates in team meetings and communicates progress and barriers with Supervisor and/or Program Manager or Department Director.

Minimum Qualifications

  • Must be a current SEP Intake Care Manager with at least 1 year of experience in role
  • Must be meeting expectations of current work
  • Bachelor’s degree in human behavioral sciences field

Attends staff, team and department meetings. Attends in-services, staffing and other meetings with supervisor’s approval. May be appointed to committees. Develops and maintains records, plans and reports. Lift and/or carry 20 lbs. Sit, stand and walk for reasonable periods of time. Maintains prompt and regular attendance. Performs related work as assigned. Ability to drive personal or company vehicle
