Your Responsibilities

As a Behavioral Health Care Coordinator, for Hillcrest Family Service’s MHC-Dubuque County, your primary responsibilities include working within the Eastern Iowa Region completing mental health screens and assessments for individuals who are incarcerated and/or involved with the court system. In addition, you will:

  • Make appropriate referrals to care, and provide short-term case coordination & follow-up to individuals
  • Provide direct community based (in the field) mental health assessments, intervention, and consultation to individuals identified by the court system as in need of potential commitment
  • Provide direct mental health services to individuals incarcerated within a county jail in the Eastern Iowa Region
  • Interact with patients, family, & support systems to reduce barriers to care
  • Participate in team delivered Care Coordination services across multiple settings (hospitals, jails, treatment facilities, shelters, family homes)

 

What You Need

  • Master’s degree in social work, counseling, psychology, or a related field and within 6 months of obtaining clinical licensure
  • Current Iowa license in social work or as a registered nurse
  • 3-5 years experience in mental health
