The Donor Family Aftercare Coordinator assists with meeting the needs of programs regarding post-donor case bereavement support to donor families. 

General Responsibilities

  • Become knowledgeable about organ, eye, and tissue donation and the transplantation field.
  • Provide bereavement and grief resources to families.
  • Facilitate correspondence between donor families and recipients.
  • Participate in planning for donor family events and celebrations.
  • Help with the planning of special projects for donor families.
  • Close collaboration and with Donor Family Aftercare Program Manager and Donor Family Aftercare Specialist.
  • Adhere to quality standards and legal compliance with regulatory and accreditation agencies (AOPO, OPTN, CMS, etc.).

Bereavement Support

  • Delivers grief and bereavement support for LifeCenter Northwest donor families.
  • Assistance with all post-donor case family follow-up, including required mailings, phone calls, written correspondence, answering donor family questions, etc. 
  • Ensures donor families have accurate, real-time access to information about bereavement support via our website, printed collateral, email, and over the phone.
  • Facilitates donor family and transplant recipient communication in collaboration with transplant centers and other Organ Procurement Organizations, ensuring timely response to requests, and quality documentation.
  • Lead in managing required mailings to ensure that they are timely and accurately delivered to donor families.
  • Support in coordinating the donor family quilt program and managing inventory.
  • Maintenance of Aftercare bereavement materials, which includes and is not limited to ordering supplies and maintaining appropriate inventory.

 

Communications and Events

  • Provides support to the Donor Family Aftercare Manager, Donor Family Aftercare Specialist, and Events Program Manager on the execution of donor family events.
  • Monitoring of Donation Community Facebook group and support with website auditing.

 

Documentation and Data Management

  • Ensures accurate and timely documentation of all family support interactions including, but not limited to, iTransplant.
  • Provides database management support to donor family programs.
  • Reviews donor family survey responses and manages data related to family feedback with the goal of improving overall program performance.

 

Qualifications

EDUCATION, EXPERIENCE & JOB REQUIREMENTS

  • Bachelor’s degree from a four-year college or university is preferred.
  • Relevant work experience can be considered and substituted for academic requirements.
  • Documented courses/programs in grief and bereavement counseling preferred.
  • Minimum 1-2 years of experience in a related field such as counseling, chaplaincy, or social work preferred.
  • Experience in a healthcare environment or related field is preferred.
  • Requires a valid driver’s license and access to an insured automobile or the ability to arrive and depart from work and work-related functions at expected times.
