Reports to: LGBTQ Health & Wellness Manager


Provide case management to health clinic patients both insured and uninsured, including but not limited to, assessment of need, service planning, reassessments, linkages and referrals to public and private services and programs, information dissemination, prescription navigation, insurance coverage navigation, financial resource management, advocacy, mental health assessment, service plan follow-up and discharge planning. Patients may represent vulnerable or marginalized populations including recent immigrants, may or may not be living with chronic health conditions including HIV.

Job Responsibilities:


  • Explain the range of available services to eligible clients
  • Conduct an initial assessment interview of individuals requesting services and conduct subsequent required reassessments
  • Develop and maintain required records, reports and statistical data
  • Maintain current knowledge on HIV-related information which may be of benefit to clients, including treatment options and their impact
  • Identify, locate and utilize all available social services and resources
  • Connect patients to Prescription Assistance Programs
  • Assist patient with insurance concerns including advocating on patient behalf with carriers
  • Recommend procedures for the implementation of client case management services
  • Assist patient with prescription transfers as needed
  • Participate in interagency councils and meetings as assigned
  • Maintain the highest level of client confidentiality
  • Attend department meetings, team meetings and supervision
  • Documents consistently, accurately, and completely
  • Will complete all annual training requirements related to the position required by DCHHS and TDSHS
  • Pull lab results as needed from electronic medical record
  • Verify patients appointments for case management services, clinic, and outside services
  • Ensure continuity of care by linking newly diagnosed HIV+ patients from Resource Center testing to Primary Care, Case Management and Behavioral Health as appropriate
  • Maintain very high level of productive communication in various formats between all stakeholders
  • Maintain routine communication with patients to ensure client remains engaged in their own care
  • Notify HIV Care Team of any suspected barriers to care that client may be facing and develop solution strategies
  • Assist with front office scheduling, answering phones, and other front office tasks as needed
  • Other duties as assigned.


Job Qualifications

  • Experience working in a community-based medical/ social services setting strongly preferred
  • Prior knowledge and experience working with clients who are HIV+ and with diverse communities
  • Experience with insurance navigation preferred
  • Bilingual (Spanish) required
  • Excellent listening, verbal, and non-verbal communication skills
  • Strong software and data entry skills
  • Intermediate/advanced Microsoft Office skills a plus
  • Available to work Saturdays as needed
