PC Household Lead- Medication Technician
Responsible for the overall care of the residents. Responsible for the development, implementation, documentation and evaluation of resident service plans. Daily evaluates residents’ conditions, assesses their needs and works with the physician to provide quality care.
Essential Duties/Responsibilities
•Works with team members to develop, implement and update a written plan of care (support plan) for each resident in accordance with physicians’ orders and assessments.
•Works with others in the Household to coordinate assignment sheets and shower schedules.
•Consults frequently with team members to ensure that resident care is being accomplished as written in support plan.
•Documents changes in residents’ conditions, open areas, doctor’s visits, special treatments, lab work, etc.
•Works closely with the Personal Care Clinical Coordinator in consultation regarding assessments, treatments, and other resident and family issues.
•Monitors behavior and changes in residents’ levels of care. Consults with supervisors, doctors, social worker, and family to deal with changes effectively.
•Communicates any resident changes or concerns to the LPN Team Leader (float).
•Responsible for medications and treatments, and reviews all required records for completeness and accuracy. Supervises and provides support to medication aide.
•Works with team members to give general supervision, guidance and assistance to each resident in carrying out that individual’s health program and to ensure that preventive measures, treatments, diet and other prescribed services are implemented.
•Assists with direct care, such as bathing, assists in the dining room, responds to call bells and participates in resident activities as able.
•Ensures completion of monthly Personal Care evaluations, admission and annual assessments, support plans, DHS medical evaluations.
•Follows admission and discharge process.
•Serves as a point of contact for families in the Household.
•Ensures that physician orders are transcribed to the support plan and eMAR.
•Assists with updating the RASP’s.
•Makes daily resident rounds of households to assess status of residents and initiates any required nursing interventions; Communicates interventions to LPN Team Leader (float).
•Serves as a member of the Interdisciplinary Team during weekly rounds.
•Assists Team Leader in getting physician communication and vitals ready for rounds.
•Collaborates with mentor to train and orient team members in the Household.
•Completes peer incident reports.
•Responds to resident call bells and alarms and supports the residents within their scope of practice.
