Summary
At Suvida Healthcare, we thrive when our people thrive. Looking for bilingual (Spanish/English) licensed, or provisionally licensed, counselors who are interested in joining a new and growing group practice in Texas. We focus on working with the elderly/geriatric population in a didactic, interdisciplinary and culturally diverse setting. The Behavioral Health Counselor (BHC) would provide individual, and family therapy services, while coordinating services with the clinical team, namely the psychiatric provider. This BHC would work with other ancillary staff to identify resources and support, as part of the treatment plan to produce better health outcomes for our patients. Essential responsibilities consist of but not all inclusive:
- Provide behavioral health counseling services
- Conduct patient assessments and establish therapy treatment goals
- Administer individual and group therapy
- Maintain thorough and confidential records of observations
- Evaluate progress and effectiveness of therapy interventions
- Provide patient with information regarding resources and therapeutic activities in the community
- Work directly with the patient, family, provider and community providers to ensure active and collaborate care
- Provide assessment, social services and counseling to patients, and their families in relation to social, psychological, financial and family situations.
- Provide individual, group and family psychotherapy
- Document therapies and treatment
- Assist with direct patient care as needed
- Initiate and conduct educational programs for staff as needed
- Market and provide community education on therapeutic services
- Develop and maintain good working relationships with outside community health and social agencies and reaches out to the community to develop new resources for meeting patient needs
- Conduct training programs with outside organizations regarding health and wellness
- Function as lead therapist regarding trauma and personality disorders, providing clinical consultation to staff
- Provide clinical consultation to clinical staff about mental health issues for various patient populations, including geriatric and substance use disorders
- Interpret and communicate necessary information regarding patients and families including social history information to staff and providers
- Coordinate planning, make recommendations, and facilitate decision-making concerning individual patient care plans especially regarding the home situation
- Plan treatments and review provider cases
- Perform other duties as assigned
