Licensed Social Worker


The Social Worker develops relationships with patients and families to provide psychosocial services and makes referrals to other existing services when necessary.  The Social Worker also participates in multidisciplinary team activities and performs other duties as assigned.

Core Responsibilities
Direct Patient Care Responsibilities

  1. Develops one-to-one relationships with patients and families as needed to provide psychosocial services such as referrals to resources.
  2. Assists in and accompanies patients/residents in the admission, transfer and discharge procedures. Assist with emotional difficulties precipitated by illness or other contributing factors.
  3. Assist with social and functioning challenges including referrals to community agencies. 
  4. Assist with assessing behavioral responses, psychoactive meds and determining appropriateness of treatment interventions as needed.
  5. Participates on Trauma Team effectively.
  6. Act as patient coordinator

 General Patient/Resident Care Responsibilities

  1. Plans, organizes, and carries out social service programs for acute care, ER, and clinics.
  2. Communicates psychosocial, financial needs and family challenges to physician and multidisciplinary team (MDT) members as appropriate.
  3. Participates in multidisciplinary team including keeping team members informed of new concerns that require attention, decisions regarding referrals, etc.
  4. Collaborates and assists with Medical Staff referrals.
  5. Chart preparation for Annual Wellness visits.

Required: Licensed as a Social Worker in the State of Idaho with a Bachelor’s Degree in Social Work or two years of social work supervised experience in a health care setting working directly with individuals with similar professional qualifications.

Preferred: Experience in hospital setting
