Position Summary:

 Responsible for organizing and delivering training including but not limited to CHW Certification, Hub Learning Sessions, Pathways Model, and HCAN data system as well as help with planning and attending work events. 

Job Responsibilities:

  • Provide training around the Pathways Community HUB Model, Pathways database, and other trainings as needed for all Community Health Workers and HUB staff;
  • Assist with startup and expansion of the Pathways HUB to additional counties;
  • Develop and maintain positive, professional relationships with each student and community organization
  • Answer non-complex questions when they arise and track training topics, dates of sessions and the number of students that attended each session
  • Attend 1-2 Professional Development session a year with pre-approval by Program Director.
  • Serve as point of contact for Community Health Workers (CHWs) regarding important client information and system issues
  • Plan and coordinate monthly Community Health Worker (CHW) meetings
  • Work closely with Program Director and other staff members.
  • Other duties as assigned

Required Knowledge, Skills and Abilities:

  • Ability to work with diverse constituents and coalition development experience required;
  • Proficient in the use of Microsoft Office Suite software;
  • Ability to learn commonly used software programs as well as specialized software required;
  • Excellent written and verbal skills required;
  • Critical thinker, able to research, design, oversee and complete projects with minimal guidance;
  • Must be a self-starter, independent worker, and able to work as a member of a team;
  • Must be adaptable and willing to accommodate routine travel within Ohio and elsewhere as required.  Hours and compensation negotiable
  • Must maintain any licensure or CEU for this position with possibility of teaching a CHW Certification class occasionally.

Job Requirements:

  • Bachelor’s Degree in health-related field or equivalent degree required; 
  • Previous experience in public health or a hospital setting required; 
  • Experience with project management required;
  • Healthcare and grants management experience desired

Essential Functions/Physical Demands:

  • Positional: Driving in accordance with job duties assigned. Infrequent standing and walking. Frequent to constant sitting.
  • Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching, handling and fingering.
  • Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
