Description

Comunilife is looking for an experienced Registered Nurse Assessor for its Adults Home program in Queens.  The RN functions as the medical/physical health lead on the Housing Provider Team responsible for assessing class members’ service needs currently and upon transition to their supported apartments. The RN assesses plans and evaluates the physical health and medical needs of class members with SMI before transitioning to community and scattered-site housing. Responsible for performing the duties of a Registered Nurse, including completing health assessments, gathering medical records to assess the class member’s functional status further, and coordinating services with other health professionals. The position also requires strong writing and computer skills to ensure compliance with funding. The RN reports to the Program Coordinator/Program Director. Essential Duties:
 
  • Work in collaboration with the Housing Provider Team and RN to formulate specific clinical summary documentation and recommendations about class members’ transition to their apartments.
  • In collaboration with the Social Work Assessor, provide professional expertise necessary to determine class members' eligibility. 
  • Perform health assessments based on reports from a doctor and records and medical provider by approving or denying services eligibility. 
  • Make recommendations related to the care needs regarding the class member’s health and safety welfare. 
  • Work in collaboration with the Housing Provider Team and the LCSW to formulate specific clinical summary documentation and recommendations about class members’ transition to their apartments. 
  • Maintain up-to-date charting and other documentation as required. Perform periodic home visits on class members to assess the client’s functioning in the community. 
  • Provide crisis intervention, including emergency hospital admission support. Actively participate in team meetings.
  • Coordinate medical/medication services with external providers, including primary care and behavioral health clinics, doctors, psychiatrist, and pharmacies. 
  • Maintain scheduled assessment appointments booked with class members within a certain number of days from an In-Reach contact with them. 

Qualifications

Education and Experience Requirement(s)
  • Bachelor of Science in Nursing degree required. 
  • Must be a graduate of an Accredited School for Nursing with an NYSED license.  
  • Certification in geriatric nursing, preferred. 
  • Previous experience in primary and behavioral health intake assessment. 
  • 5+ experience working with older adults with serious mental illness preferred.
  • Excellent organizational and communication skills, both written and verbal. 
  • A strong strengths-based, recovery
  • Working knowledge of Microsoft Word and Excel, plus proficiency in health-related computer applications. 
  • Knowledge of the community mental health system and various community resources and understanding of entitlements and enrollment requirements related to Managed Long-Term Care and Certified Health Home agency.
