Position Summary
The Licensed Clinical Social Worker will assess the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs and help provide both the counseling and casework needed to meet these goals. This position independently evaluates patient’s psychosocial needs to promote quality care and cost-effective outcomes for patients requiring complex outpatient care.

Schedule:
Flexible, may require some evening and weekends

Primary Duties and Responsibilities
Interview, coordinate and plan programs and activities to meet psychosocial needs of patient
Provide consultation and education
Work with multidisciplinary team to ensure overall health and well-being of patient
Conduct intake and assessment of patients available resources and needs
Formulate assessments and direct treatment accordingly
Develop an individual-based and/or group-based treatment plan, including medical, psychological, financial and other life issues
Maintain quality assurance of the program
Maintain confidentiality at all times
Participation in Northeast Community Clinic’s Continuing Quality Improvement (QI) program.
Contributes to program planning, evaluation, report preparation and development of articles.
Performs other tasks as required or as assigned by the Program Director and Medical Director.
Maintain a positive working relationship with staff.
Other duties as designated by the Medical Director or Executive Director

Qualifications

Minimum Requirements
Microsoft Office (Word, Excel, Outlook)
Excellent Communication Skills
Excellent Customer Service
Bilingual English/Spanish preferred
Ability to work as a team player and work independently
Reliable transportation
Must be able to travel from facility to facility
This position requires 10 % travel outside the local area.

Required Education/Experience and/or Licensure/Certifications
Masters in Social Work (MSW) from an accredited college or university
Current CA LCSW License
Experience in outpatient clinic setting preferred
Commitment to goals and philosophy of Northeast Community Clinic
Current CPR and/or BLS Certification
Valid State Identification

Physical Requirements and Working Conditions
OSHA Category 2 Involves no regular exposure to blood, body fluids, or tissues, but may require unplanned tasks that involve exposure to blood, body fluids, or tissues.
The work is majority of the time sedentary in nature.
The employee is regularly required to communicate, frequently required to use repetitive motions, move, remain stationary, regularly push, pull and lift up to 20 pounds and occasionally push, pull and lift up to 40 pounds.
Must possess mobility to work in a standard office/clinic setting (in some cases both settings) and to use standard office/clinic equipment, including a computer, stamina to maintain attention to detail despite interruptions, read printed materials and use a computer, and communicate in person and over the telephone.
