Position Summary

The role of the Licensed Practical Nurse reports to the Nurse Supervisor. This position, under direct supervision of a registered nurse, is responsible for providing direct patient within the scope of practice. This position will be responsible for completing home visits and ensuring all related documentation is completed in a timely manner.

 

Essential Position Responsibilities

  • Chart observations of the patient’s condition at each visit
  • Complete evaluation tasks, including reviewing medication and vital signs
  • Complete written reports on patients in a timely manner
  • Provide thorough education for patients and families on proper home health care procedures and strategies
  • Partner and coordinate care with members of the patient’s care team
  • Partner with registered nurse regarding patients condition or changes in medication

Qualifications

  • Education: Must be a graduate of an accredited nursing program
  • Licensure and Certifications: Licensed practical nurse and CPR required. Valid drivers license and auto liability insurance, required.
  • Experience: Minimum of 1 year experience in a clinical nursing setting, required.

 

Work Environment: This position will spend 80% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50 lbs. This position will spend 20% of time sitting while performing work in a standard office environment. This position will require travel to patient homes and will be exposed to communicable diseases and bodily fluids.
