Recovery Network of Programs, Inc. (RNP), is a private, non-profit, social service agency which has been serving the Greater Bridgeport Community and Fairfield County area since 1972. RNP is dedicated to helping those living with substance use, mental health disorders, and homelessness. New Prospects - located in Bridgeport, Connecticut is uniquely positioned to offer gender-specific co-occurring enhanced intensive residential services to adult men and women. This program is designed to provide services for individuals who are experiencing difficulties stabilizing their substance abuse and mental health issues and seek short-term intensive treatment of approximately 30 days.

 

HOURS: Monday to Friday 8:30am - 4:30pm 

 

CORE FUNCTIONS

 

  • Provide telephonic and in person support to clients and ensure access and connection to community based behavioral health services, identification of any barriers, and assistance with barrier resolution.
  • Maintain case load of clients in addition to providing After Care services to all clients of the program.
  • Coordinate referrals inter-agency and ensure the transition of medicated assisted treatment is successful prior to clients’ discharge
  • Provide collateral contacts to after care community collaborators and confirm releases of information are in place to ensure a smooth transition of information and referral, and that clients have successfully connected with their aftercare placement
  • Document all contacts; work collaboratively with the clinical team to assist in identifying and placing clients in the appropriate aftercare services that offers interventions tailored to their diagnoses
  • Assist with the development and implementation of data collection/reporting mechanisms, and ensure timely and regular reporting of all outcomes of interventions and referrals as required by funding partners and accreditation bodies
  • Work closely and in a well-coordinated manner with colleagues and community-based providers to ensure clients and their families receive access to timely, high quality care and services, including follow up to ensure connection.
  • Identifies, analyzes, and resolves client after care problems to foster significant improvement in their care
  • Participates in interdisciplinary continuous quality improvement activities on an ongoing basis; participate in weekly clinical meetings and individual supervision
  • Demonstrates a commitment to improving the quality of care provided to clients
  • Serve as advocate for clients in obtaining necessary services.
  • Adhere to all standards as it relates to documentation requirements.
  • Observe meals, laundry and smoke breaks
  • Assists with intakes of new clients
  • Assists with coordination of daily discharges
  • Assumes all other duties as deemed necessary by the supervisor.

CORE COMPETENCIES

  • Bachelor's degree, such as a BSW, or Associates degree with completed course work in human services, or Certified Recovery Coach and/or Community Health Worker certification
  • Experience with providing client advocacy, client navigation and clinical support services to help clients access community based behavioral health services
  • Experience working with interdisciplinary teams
  • Experience using an electronic health record Microsoft Word, Office, Excel, Outlook skills
  • Knowledge of behavioral health (mental health and substance abuse) conditions and related needs
  • Knowledge of principles and techniques of social service case management services
  • Knowledge of the physical, psychological, social and economic effects of substance use, mental health disorders and crisis intervention. 
  • Ability to gather, analyze and evaluate information pertinent to client care. 
  • Knowledge of community resources. 
  • Considerable knowledge of federal and state laws and regulations regarding client confidentiality. 
  • Ability to establish and maintain cooperative professional relationships. 
  • Excellent communication skills. 
  • Ability to project a professional image.

 

CERTIFICATES, LICENSES

  • Incumbents may be required to secure professional certification and/or license. 
  • Valid Connecticut driver's license is preferred.
