Lower Lights Christian Health Center (LLCHC) transforms the overall health of Central Ohio, serving one individual at a time. Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral health care, 340B pharmacy, nutritional assistance programs, and more.Working hours are Monday - Fridaywith occasional Saturday morning coverage.

JOB SUMMARY

The ideal candidate is an organized, collaborative professional with strong planning and administrative skills. The ideal candidate has an understanding of and compassion for those who are struggling with SUD.

DUTIES

RESPONSIBILITY 1. Treatment Assistance

  1. Maintains MAT cell phone to process call, schedule patient, coordinate services to engage pt in addiction treatment program.
  2. Coordinates with PCP on patient care.
  3. Refers patient to supportive services to supplement treatment and counseling.
  4. Meets weekly with Freedom Recovery and Maryhaven to coordinate care of Marysville patients participating in programs.
  5. Maintains a collaborative work relationship with all program staff.
  6. Documents completely and clearly in the patient’s EHR chart.

RESPONSIBILITY 2. Planning & Grant Coordination

  1. Completion of GPRA assessments for all MAT patients in Union and Franklin counties.
  2. Utilize systems for tracking all referrals and provider patient panels.
  3. Daily monitoring of providers schedules and contacting patients to ensure follow through with recommended appointment schedule.
  4. Oversees all program stats and data tracking for monthly reports, quality and grant reporting.

RESPONSIBILITY 3. Coordination of Care

  1. Coordinates linkage referrals from OSU, Mt. Carmel, Ohio Health systems to connect patients from inpatient and the ED to LLCHC addiction treatment program.
  2. Communicates to care team in a timely manner regarding patient status.
  3. Coordinates scheduling of random urine drug screens.

Qualifications

  • Associate degree in social services and two years of work-related experience preferred.
  • Knowledge of and experience in substance use disorder and related issues and understanding of recovery models including harm reduction and relapse prevention.
  • Experience in successfully working on an interdisciplinary team.
  • Proven ability to develop and maintain systems of communication, data management, and grant reporting.
  • Interviewing skills, time management and organizational skills, critical thinking skills, conflict resolution skills and ability to work independently.
  • Ability to problem solve, exercise sound judgment, define problems/barriers, collect data, make decisions, maintain confidentiality.
  • Communicating effectively in both written and verbal form.
  • Knowledge of computer system, electronic medical record, data collection, excel, tracking spreadsheets.
  • Ability to function in a team environment and a mature understanding of cultural diversity.
