Job Position: Health Home Supervisor

With strong support from the Rendr leadership team and the lead Health Home of Community Healthcare Network, the Health Home Supervisor will create and oversee the comprehensive care coordination and care management services for adults over 21 years of age assigned to our health home. This position will supervise several health home case managers to support individuals with chronic medical conditions to achieve short- and long-term health care goals and program objectives.

 

Essential Functions

  • Provides regular individual and group supervision, treatment team meetings, case conferences, in-service training, and other staff development activities
  • Hires, trains, and evaluates health home case managers
  • Assigns and monitors enrollments to the program to ensure that appropriate assessments are made, clients are connected to the appropriate services, and appropriate discharge planning is provided
  • Reviews the teams caseloads and charts on a regular basis and ensures that all documentation and billing meet the goals, regulatory requirements. and quality standards of the program
  • Supervises the development and implementation of comprehensive care plans and assessments to ensure that they are maintained in a timely, accurate, and complete manner
  • Provide coverage and care coordination for patient caseloads, as needed
  • Supports care coordination by staff to effectively navigate complex or difficult cases and the prioritization of needs in a patient-centered way
  • Notifies leadership of any serious incidents, including but not limited to allegations of abuse (including the reporting of child abuse or neglect)
  • Coordinates with the Quality, Corporate Compliance, and other appropriate Departments to prepare for audits; develops initial plans of corrective action, incident reports, and program evaluation reports; and addresses areas that need improvement
  • Communicate and collaborate regularly with patients, physicians, community agencies, and office staff with respect to patient services and compliance with care plans
  • Seek and develop partnerships with community-based organizations as needed to provide appropriate care to enrollees
  • Attend regularly individual or group training activities and program-related meetings on or off-site to stay informed of all federal, state, and city requirements of the program to maintain compliance
  • Learn to use multiple health information technology platforms for effective case management of the health home program
  • Perform all other duties, as assigned

 

Qualifications

  • Licensure by the NYS Education Department as a mental health practitioner, social worker, or psychologist preferred
  • A Master's degree in a professional mental health or social work discipline preferred
  • Knowledge of Health Home operations, including state and federal regulations and compliance
  • At least one year of supervisory experience preferred
  • At least three years of experience in a care management, ambulatory care, social work, or behavioral health clinic setting
  • Effective critical thinking and problem-solving skills
  • Strong writing, communication, computer skills, and documentation skills
  • Ability to multitask, stay organized, and have excellent time management skills
  • Bilingual in English and Chinese preferred
  • Traveling is required
