  • If your child is under the age of 18, this is a temporary policy change allowing you to be a Habilitation provider for your child and may end at any time as determined by the DDD.
  • If a parent is working from home and is already being paid by his or her employer during the time a child is scheduled to receive services, then the parent cannot also be paid as a Direct Care Worker for the child.
  • A parent may NOT provide Respite to their own child.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide services and supports as identified in each clients Individual Service Plan in the domains of daily living skills, self-help, and socialization.
  • Use professional language at all times while working with supervisors, other staff members, serviced individuals and their caregivers.
  • Assist each client by providing both formal and informal training in activities of daily living activities such as dressing, bathing, eating, cooking, cleaning, etc.
  • Provide client training on new skills to increase independence and community safety when outlined in the individuals ISP.
  • Provide training and assistance with self-administration, and administer all ordered medications according to State of Arizona approved guidelines only after all necessary medical documents have been reviewed and approved by HOPE Group.
  • Attend mandatory trainings, staff meetings and direct supervision sessions with their supervisors in addition to their regularly scheduled client sessions.
  • Complete all appropriate documentation such as progress notes, individual work records, medication administration records and other work-related documents.
  • Complete all time sheets; leave slips, accident/incident reports, etc., within their stated due dates.
  • Ensure a healthy and clean work environment.
  • Observe for signs of illness, changes in health, or signs of abuse and report to supervisor and/or Human Resources.
  • Provide behavior support and crisis intervention as needed and outlined by the ISP.

  • Ability to define behaviors, collect data, and understand client needs.

QUALIFICATIONS & POSITION REQUIREMENTS

All personnel hired by HOPE Group shall be fingerprinted as a condition of employment, except for certified personnel who as a condition of certification are required to have a valid fingerprint clearance card. All other personnel shall obtain a state level fingerprint background check.

EDUCATION AND/OR EXPERIENCE

  • Valid State ID; must be at least 18 years of age
  • Current CPR and First aid certification (originals must be provided for copy and file)
  • Proof of training and proficiency in A.A.C. Title 6, Chapter 6, Article 9
  • Knowledge, skills and experience sufficient to carry out the requirements of the position
  • Fingerprinting, fingerprint clearance and disclosure statement regarding criminal record signed by applicant
