We are looking for a Clinic Registered Nurse (RN) – Infectious Disease to join our clinics. This position involves HIV program-specific clerical and clinical duties, disease management support, patient education, HIV coordination of medical care and resources as directed by the HIV provider. They ARE Responsible for performing within scope of practice and in a professional leadership role to promote excellence in patient care and customer service for the HIV program participants throughout Community Health Care. Provide clinical nursing care for HIV program patients to include specialized intake, appointment scheduling referrals, tracking, and blood draws. Conduct initial pre-screening of clients with an intake assessment. Work with eligibility and case manager to ensure patient is ensured and qualifies for services within the program. Provides patient education to include medication administration and adherence, side effect management, safe sex practices, risk reduction counseling and lifestyle education. Communicates with TPCHD with positive STDs, disease investigation, HIV diagnoses to ensure timely treatment, evaluation and follow up scheduled. Accurately and completely document patient data, interaction, and response to care in EMR and routinely audit provider team’s medical records. Plus, other duties as assigned.

Qualifications

Current Washington State RN license.
