Pagosa Springs Medical Center’s Clinic has an opening for a Full-Time Integrated Behavioral Health Care Provider (IBHCP). This position will not be hired before a grant funding contract is signed (estimated to be between November 2023 and January 2024). Must be licensed in Colorado as a Licensed Clinical Social Worker. PSMC will provide professional liability insurance coverage and a CME allowance of $6,000 annually (gross and subject to taxes) for full-time. Compensation range is $28.47 to $41.40 per hour depending on experience. PSMC and Provider each contribute 6% of earnings to a 401a Retirement subject to I.R.S. contribution and compensation limits and five-year vesting. Voluntary participation in 457 plan. Job Duties
This position provides integrated behavioral health care to Rural Health Clinic patients. This type of care is delivered with a warm hand-off during the clinic day or is a scheduled visit. This position also provides typical social work supportive to patient needs. The IBHCP uses specialized education, discretion and judgement to perform the following: 1. Assess the patient’s physical, psychosocial, cultural and spiritual needs, utilizing evidence-based screening tools (PHQ9, GAD7, CSSRS, etc.); 2. Assist in the coordination of appropriate services, resources, and referrals; 3. Advocate and provide support to patients and families having difficulty coping effectively with changing medical conditions and other social determinants that may affect their health and well-being; 4. Diagnose and provide short-term evidence-based behavioral health therapy in the outpatient Rural Health Clinic; 5. Refer to a higher level of care when required for diagnosis and medication management; 6. Facilitate remote therapeutic sessions with offsite providers (Psychiatrists, Psychiatric NP, etc.) via telehealth when needed and act as a liaison between the specialist and the primary care provider. Duties may be changed as clinic needs are modified.
