The Patient Navigator (PN) plays a key role by providing resource support to patients as they navigate the health care system. The PN is a member of the health center's primary care team, and partners closely with different providers, patients, and outside organizations. He/she is responsible for increasing patients' access to health care and community resources/social services, facilitating communication between patient and care teams; and providing culturally appropriate health education and self-management support.
Key Responsibilities:
- Coach patients in effective management of their chronic health conditions and self-care
- Assist patients in identifying the barriers to health that they identify and prioritize
- Document activities, care plans, and outcomes in an effective manner consistent with organization's policies and procedures
- Provide ongoing follow-up, basic motivational interviewing and goal setting with patients
- Build and maintain positive working relationships with clients, providers, supervisors, and staff
- Continuously expand knowledge and understanding of community resources, services and programs provided
- Calling and tracking patients who are overdue for a visit, or have missed a visit
- Confirming access to appointments
- Arranging to add interpreters to telehealth visits, if needed
- Assisting patients in scheduling visits and provide guidance on visit requirements
- Providing education for patients on-request by providers
- Providing community-based education through outreach and engagement with local, community-based organizations
- Screening patients for social determinant of health and be knowledgeable to refer to community resources
- Responsible for maintaining registries of patients for outreach
- Attend team meetings
