You will be responsible for the coordination of services and pursuit of excellence in integrated care for our patients. Focus on Patient Education, Assessments, implementing and monitoring care plans, triaging calls and ensuring Quality outcomes.

Graduate of accredited school of Nursing, BSN strongly preferred, Demonstrated proficiency with EHR, with EPIC a strong plus.. The ideal candidate will also have worked in an outpatient setting and experience working with underserved populations.
