We are looking for an Outreach Worker Referral Tracker to join our clinics! This position tracks all referrals for specialty care and diagnostic tests to completion. This includes ensuring appointments are scheduled, kept, and retrieving progress notes and results from specialist or diagnostic imaging center. Our Outreach Worker Referral Tracker’s work with Outreach to ensure barriers to scheduling are overcome so timelines are met, contact the patient when issues arise and to ensure all needs for urgent appointments are met, plus ensures full completion or cancellation of every referral generated. In addition, Contacts specialty and diagnostic office when specialist specifies, they will schedule appointment and makes note of appointment date and assists patients to overcome barriers to complete appointments. Plus, other duties as assigned.

Qualifications

High school diploma or equivalent combination of education and experience.
