BASIC BEHAVIOR RESPONSIBILITIES:
- Understands Keystone’s organizational chart and uses appropriate professional protocol; displays kindness and respect in all communications and interactions with stakeholders, supervisors, co-workers and patients. Performs duties as prescribed by Supervisor(s).
- Commits to upholding Keystone’s Vision/Mission/Core Value statements; understands and complies with Keystone’s policies and procedures; promotes a Recovery Oriented System of Care including actively supporting integration of Medication Assisted Treatment across all Keystone programs; maintains the commitment to ethics and total quality, and complies with all applicable rules and regulations including state and federal confidentiality laws.

- Ability to demonstrate recovery expertise – including knowledge of approaches to support others in recovery and dual recovery – as well as the ability to demonstrate his or her own efforts at self-directed recovery.
- Conducts care management for patients in collaboration with the clinical team throughout the treatment process.
- Assesses patient needs and provides access to housing, education, employment, child care, transportation, legal services and any other needs, including Medication Assisted Treatment that would help the patient transition smoothly into the community.
- Maintains effective communication with community partners through staffing and case review.
- Maintains patient record in compliance with all governing bodies to include DAODAS, DHEC, CARF, DHHS (Medicaid) and DSS.
- Requires regular and reliable attendance.
- Remains current by attending Safety Trainings including driver training, receive an annual Tb skin test or its equivalent, and mandatory 20 hours training for re- credentialing annually.
- Proficiency with MS office computer software applications and/or Medical Records software applications, emails, etc.
