This is a temporary position through December 31, 2025.
For more than 30 years, the Adult Day Center has been there to help care for loved ones who are elderly and frail with Alzheimer's or another form of dementia, Parkinson's, or the vestiges of a stroke. Often, family members are left with uncertainty and helplessness about their role as caregivers and are overwhelmed by the competing demands of daily life, earning a living, and providing full-time care to their loved one.

POSITION SUMMARY
To ensure the overall safety, security, and well-being of elderly clients of the Adult Day Center (ADC). The Direct Care Staff will lead activities that engage and stimulate older adults with memory loss. This role will also assist clients with activities of daily living, including assistance with eating, showering, toileting, and basic custodial care.
PRIMARY RESPONSIBILITIES

Assists with the client’s safe arrival and departure to the ADC, including signing them in and out on the daily roster.

Prepares, implements, and leads program activities as assigned on a daily basis.

Escorts and assists clients with personal custodial needs, including showering as needed.

Assists with meal and snack preparation and service; provides clients with needed assistance; monitors for general appetite and safety; and provides adequate fluids to maintain hydration.

Monitors, reports, and documents clients’ meal intake, restroom schedule, health, behavior, hygiene changes, and program participation.

Report any change in the client’s health or behavior to the Supervisor based on observations and activity project selections.

Communicates with family members as appropriate.

Responsible use of agency property and supplies.

Assists in maintaining the cleanliness of the facility.
ESSENTIAL JOB FUNCTIONS

Ability to assist with the client’s safe arrival and departure at the ADC.

Capable of assisting with meal and snack preparation and service; provides clients with any needed assistance; monitors for general appetite and safety; and provides adequate fluids to maintain hydration.

Ability to safely maneuver wheelchair-bound clients (on upward and downward slopes), and/or those using assistive walking devices, involving pushing and controlling the device.

Ability to transfer clients who use wheelchairs into and out of their chairs and into and out of vehicles, requiring lifting to 35 pounds unassisted*
OTHER RESPONSIBILITIES
Due to changing program demands, funding source expectations, and/or changes in agency policy, as assigned, all staff may be expected to perform other duties.
POSITION QUALIFICATIONS
Educational Requirements
Must be 18, with a high school diploma or GED (some college preferred). A Certified Nursing Assistant (CNA) certificate is strongly preferred.
Experience
A minimum of one (1) year of experience working with older adults with dementia, Alzheimer’s disease, disabled, or other at-risk populations.

Must obtain and maintain a CPR and First Aid card upon hire.

Must complete Health Screening Report within 3 days of hire (proof of TB clearance must be kept current every 4 years as required by the County of Los Angeles).

Fingerprint background clearance required.
*Assistive mobility equipment is not available on-site. Due to the physical nature of the work involved, the job duties must be performed safely so as not to create overexertion injuries. This position requires transferring and maneuvering individuals. Must be able to maneuver up to 35 pounds.
Constraints
Wise & Healthy Aging is funded through various government contracts, as well as private and public fundraising efforts. The continuance of all positions depends on the organization’s ability to maintain sufficient funding to support all positions.
