POSITION SUMMARY (Part-Time)
The Care Coordinator will provide efficient, comprehensive, and effective care management services to older adults and disabled residents. These services include a comprehensive assessment of the clients, development of a written care plan with measurable outcomes, arrangement, and coordination of services, and client follow-up to monitor the effectiveness of the services arranged and assess the need for additional services.
PRIMARY RESPONSIBILITIES
- Ensure high-quality client service delivery.
- Conduct client intake screening and in-home assessments using program-specific forms to identify client needs and confirm eligibility.
- Travel to clients' homes and other sites, as needed, up to 50% of the time.
- Conduct a comprehensive in-home assessment and write a biopsychosocial assessment. Complete paper documentation including progress notes and billing related to all CM protocols.
- Develop a written Care Plan with measurable outcomes to address and implement an agreed-upon action plan for client needs and complete it based on contract requirements.
- Identify and mobilize appropriate resources to address client needs and resolve problems.
- Arrange and coordinate services on behalf of the client.
- Maintain contact with the client to monitor the effectiveness of the service arranged for the client and assess the need for additional services.
- Write accurate, thorough, specific client assessments, letters, emails, and other communications. Complete timely, accurate, and detailed documentation for client files.
- Ensure service recording, including accurate billing, is entered into software systems on a weekly basis.
- Provide direct client service to 20-25 clients.
- Prepare for and participate in weekly Supervision meetings with management and other staff.
ESSENTIAL JOB FUNCTIONS
- Ability to perform biopsychosocial assessments of clients using appropriate techniques.
- Ability to document client information and maintain client boundaries and confidentiality.
- Ability to visit clients’ homes using personal vehicle or other means of personal transportation.
POSITION QUALIFICATIONS & EDUCATION REQUIREMENTS
- Licensed Clinical Social Worker (LCSW)
- Master’s degree in social work, Gerontology, Public Health, Human Services, or related field required.
- Minimum three years of experience working with older adults.
- Fluency in English (bilingual in Spanish strongly preferred).
- Previous experience working with an ethnically and economically diverse population.
- Previous care management experience is strongly preferred.
OTHER REQUIREMENTS
- Must have reliable transportation, a current driver’s license, a clean driving record, and proof of auto insurance.
- 1+ years of experience with MIS tracking system, a Plus.
- Travel is required within Los Angeles County.
- Must pass a criminal background check.
- Medical billing experience
